Doctor Constantini explained as follows:

"we use thiamine hydrocloride intramuscular phials or tablets taken by mouth. Generally, with the oral therapy we start with 2 grams each day to be taken once a day in the morning, in order to increase the dose later consequently to the results obtained. The oral therapy is as effective as the intramuscular one. Some follow-ups of the results are always necessary"........"in the experiment we used thiamine hydrochloride. Thiamine administered intramuscularly is 100-140 times stronger than the equivalent oral dose, depending on the personal patient's absorption. So, for example, to have the clinical effect of 1 im injection once a week, we have to administer 2 grs orally each day for 7 subsequent days."

Addressed to me, Roy, Doc Costantini's recommend: “For your situation 1 intramuscular injection twice a week (or an oral dose of 4 grams each day, two tablets in the morning and two at lunch time) should bring to the complete regression of the symptoms in 1 or 2 months.”

Note the dose in the morning and again at lunch time.

No I am doing the oral protocol. Taking the second dose at dinner has not interfered with my sleep.

I looked over my documents and found no explanation. So, if we look at biology, how long does it take for a dose to pass through the digestive tract and excess pissed off? Doc must want a sustained high dose for these first hours of the day. I admit missing that dose timing recommend from the get go. I felt it most important to spread the amount over two equal doses over 24 hours.

I want to wait for three more weeks before 'monkeying' with the dose schedule but I may just start morning and lunch time tomorrow.

I have been doing morning and evening doses also.

Tomorrow I start (as he recommends) 2 x day (morning 2g and lunch 2g) as Solgar Vitamin B1 (as thiamin HCL) 500mg tablets.

I wrote directly to Dr. Constantini and asked him why lunch instead of dinner. His reply was that if the second dose is taken at dinner it can interfere with sleep, as Astra7 said. In my case it does not seem to interfere with my sleep so I will continue to take my second dose at dinner.

Of course, bears are very good at sleeping, so your mileage may vary.

Hello, Park_bear. How do you contact Dr. Constantini. Does he have a website. I couldn't find anything. What other good supplements will go along with B1 to help with tremors. My dad tried Sinemet and Rupinirole, nothing helps with tremor.

If you peruse this patients' forum you will find various other suggestions with varying degrees of evidence of efficacy. Some people report help the form of B3 known as "NR". Others report improvement from using mannitol. Citicholine, CoQ10, and glutathione have all been been reported as helpful. NAC - N-acetylcysteine - was reported as helpful in a study that used both IV and oral formulations. Effectiveness of it in strictly oral use has been brought into question.

I was taking Thiamine Mononitrate for 7 months and been taking Thiamine HCL for over 2 months. I can tell you that not only my PD progression stopped but I have significantly had improvements in my symptoms.

The only symptom been resistant is my dystonia that I think related to my magnesium deficiency which I am trying to fix.

I have visited Dr Costantini 3 times in Viterbo,Italy since last May and been under his care since then.I am taking 3grs Thiamine HCL divided into 2 times a day morning and afternoon.I asked him why we should not take it at night and he said that taking Thiamine after 5 PM may affect our sleep.

I just hope everyone saw Dr. C's recent email where he stated that some patients have had violent reactions to B-1 injections and his research shows that when thiamine is effective what it does is improve overall quality of life and not reverse or slow progression.

Hello ion_ion. I'm wondering what effect the Thiamine HCL 2g / day is going for you since you started 3 mos. ago? We want my husband to start on B1. So do you take 1g in morning and 1 g at noon? Thanks!

Hi Elaine! I started with 1g about one hour after breakfast and 1g one-two hours after lunch (two Solgar pills of 500 mg each which I chew and swallow with water). After a week I started to see a big difference in mental and physical energy. But one time I was too agitated and stopped for few days and resumed in lower dose trying to make a compromise between the level of energy and intensity of tremor. Right now I take 3 pills (1.5g/day) - first one hour after breakfast, second one hour after lunch and the third two - three hours after lunch. Bigger dose more energy and bigger tremor, lower dose lower energy and less tremor - trying to find the optimum spot.

But as final result helps a lot with my brain power.
