If you have been diagnosed with a long term condition in Essex, or know someone who has, Essex and Southend LINk want your views on the information given to you.

WHO ARE ESSEX AND SOUTHEND LINK?

An organization that works with health care professionals and service users, to improve health and social care in the area. They provide a way to say what you think and to make suggestions. LINk aims to make sure all views are heard.

Their website is essexandsouthendlink.org.uk

WHY IS THE RESEARCH BEING DONE?

There are concerns that following diagnosis of a long term condition, the sufferer (or someone close to them), are given insufficient information and/or support.

The project aims to identify how information should be given to enable a better understanding and management of the condition after diagnosis.

The findings of LINk’s report will be presented to North East Essex PCT. Recommendations may be implemented based on the report results.

To take part in the research:

Complete a questionnaire and return it by the end of February 2011.

1. Complete the survey online at:

surveymonkey.com/s/longterm...

2. Download a copy of the ‘Long Term Condition Questionnaire’ at:

essexandsouthendlink.org.uk...

3. If you would like a Long Term Condition Questionnaire to be sent to you or if you require any more information then please contact:

Annie Sayer BSc MRes, Project Researcher, annie.sayer@essexandsouthendlink.org.uk
