Hi everyone.... first, I just wanted to let you all know how helpful it has been to read your posts and hints for recovering from this surgery. My husband is one year out. As many of you, he has had several stretchings of his pylorus, and had it cut in October, which did provide several months relief. Unfortunately, he is having stenosis again, and the doctors are recommending a G-POEM. Anyone have any experience with this? Thanks to all of you.... I really can't express how much you all have helped me be a better caregiver to him, and keep my sanity❤️❤️❤️

Stenosis is a narrowing of the affected part, I believe. POEM is per oral endoscopic myotomy which is where the surgeons cut the muscles within the wall of the oesophagus to release the pressure exerted by the muscles that clamp the lower oesophageal sphincter tight shut, the alternative being conventional surgery to cut those muscles. So G POEM is more or less the same procedure on the next sphincter down, the pyloric sphincter, with the equipment and techniques being much the same as POEM. My question to the surgeon would, I think, be related to whether the previous procedures would affect the chances of success, whether they are sure that it is indeed the muscles in and around the stomach wall that are causing the problem, how will they judge how much to cut so that there is no upward flow of intestinal contents afterwards, and, perhaps, how many they have performed before.

I personally have not heard of this, but I can understand the logic of what they are suggesting, and you will appreciate this this is all non-qualified speculation on my part which should not stop you trusting their expert surgical judgement
