A year or three ago, Medway, who posted on this forum, had a similar operation. The technique is not done very much nowadays. Sadly, Medway is no longer with us, but the surgery itself went OK as far as I remember.

Thank you Alan. I am sorry to hear about Medway. I had seen his posts. It is rare due to technical aspects. I have a similar decision to make roux or merendino. I do not have cancer but serveral failed fundos and its salvage time. What was your op Alan?

Personally I had a repair of a ruptured oesophagus (non cancerous). I am sympathetic with patient consultation about these procedures but it is also quite a technical, specialist issue for experienced surgeons and one has to listen very carefully to their judgement about any longer term issues.

Yes Alan, very true. It's difficult when one recommends something and another has a completely different opinion. I have have many tests and consultations from the best and they boil down to the Atilla Csendes salvage op' of Hill repair, selective vagotomy, 70% resection and roux 60cm or the Merendino, very doubtful vagals can be spared so pyloroplasty likely. In case 1 the risks of roux stasis and stasis of the remnant are there. With Merendino, alkaline reflux is a possibility, plus all the usual surgical, post op' risks.

To be fair it is a very specialist surgical area that is pretty rare. I wish you well for it all.
