I went to the hospital on 5/23 I was 4 weeks 1 day & they did an ultrasound and seen completely nothing and my hcg was 521 they told me to come back 5/26 so I went back an a gestational sac was visible very very small but no yolk sac or fetal pole an my hcg went up all the way too 2,524 Has anyone had this happen before and is it just to early for yolk sac or fetal pole to show ?

Yes, very very early. You should be scanning from 7 weeks to have confirmation of heartbeat and fet pole.
