Can phimosis come back?

I have recently been listed for a preputioplasty and i'm now wondering if the condition may return at some point, which will then require a full circumcision. Online documentation of the preputioplasty is scarce and lacking in detail. most posts of people praising the procedure are from immediately after their operation, but no one seems to have anything to say regarding what happens a couple of years down the line.

on another note, how do i properly use steroid cream? i have prescribed diprosone 0.05% cream (not over the counter) and i'm still unsure as to how to apply it properly.

Do i put it on the outer or inner skin? do i stretch before or after applying the cream? should i stretch at all?

it has helped massively as i am now able to retract while soft, although it is still tight behind the head and retraction is impossible while hard. but there has been no further improvement from there.

i have no idea what to do :( why can't i just have a functioning sausage?

Cream won't work for everyone. Usually people that have issues pulling down foreskin while flaccid won't have much success without a circumcision. But if it helped so far, it might help with the rest it just will take longer. The issue with stretching ... the skin is elastic ... so it can loosen up, but it will also tighten up again if you do not keep "using it" as in pulling it over your penis. But as soon as you get it "over", and keep getting it "over" regularly, theoretically it should not revert back. However I have nothing to back this theory up other than guesswork.

Are you opposed to a complete circumcision ? Coz once the skin is gone there is nothing to retract to being as before if that is your worry.

i retract it pretty much every day, but some people say that you should put two fingers in the prepuce and pull it outwards to stretch the phimotic ring.

and yes i would like to avoid circumcision unless there's something life-threatening going on down there.

At my work in the hospital I've seen many guys who had a preputioplasty after awhile come back for a circumcision because the foreskin had tightened again. A friend had it too.

A preputioplasty involves cutting the skin. When the skin heals it forms scar tissue which is non-elastic. Thus requiring a full circumcision. When I was a kid a friend had a "partial" circumcision whereby just the very tip of his foreskin was cut off (the puce colored wrinkly folds on the very tip). This resulted in a non-elastic ring of scar tissue that would not retract over his glans and required a second circumcision to remove the scar tissue.

So, when you stretch the foreskin using fingers or a device the cream interferes with the inflammatory process that reverses the stretching has done. If you use the cream beforehand I'd imagine that it would only hinder your ability to properly stretch the skin. What you are doing is using your body's natural function- mitosis. In smaller terms you stretch the skin and your body realizes that there's extra space that it can fill it does so- over time this widens the foreskin and an approximately works 80-90 percent of the time. As long as you use your foreskin and retract it like you need to (showers masturbation, urination etc) or repeatedly damage it-it should not reverse the effect. What you do when you go in for that surgery they make an incision in your foreskin and separate each side of the incised foreskin and suture it like that- it becomes an extension of your foreskin. I have never seen someone come in for A second operation to get circumcised or another prepucialplasty after an initial foreskin-sparing surgery and I work as a practicing urologist.

are there any other procedures? i know of the dorsal slit but i haven't heard of anything else. can't they just take some of the skin off, like a partial circumcision?

There is, you can have a partial circumcision which removes the part that becomes "tight" in phimosis. Removing that should get rid of any reocurrance permanently. There is a downside though- the skin covering your glans may not stay on and retract at awkward times, but it does let you keep the protective effects of the foreskin(preventing glans from becoming dry and irritated and keeping the shiny effect your glans have because they have no calluses like a fully circumcised penis from birth). Either choice you choose I would recommend leaving the frenulum and as much inner foreskin as possible because of the fact that it is sensitive and potentially erogenous depending on the person.

When discussing my circumcision with my doctor he said this was a procedure he was never prepared to offer due to its lack of longevity and the chances of the condition returning

Exactly my experience! Some urologists seem to promote this procedure because of anti-circumcision resentment or out of profit (it's more expensive than a circumcision).

Hey I was in exactly the same boat as you I had the Preputioplasty surgery 10 years ago and foreskin was still tight with an erection but retracts fine while soft went back to the drs 2 years later after trying creams and stretching exercises and had the same surgery again 2 years later and since then have had no problems with a tight foreskin even while erect. Coincidentally my current BF had the same issue and he went down the circumcision route second time round this was before I met him and he now wishes he could go back in time and have the Preputioplasty again not the circumcision I only have 2 very small scars which are barely noticeable. It seems that there are quite a few drs and people out there who are pro circumcision. Hope your dr gives you the opportunity to make the desicion.

wait so you had two preputioplasty operations two years apart, and then after that no problems?

also from what i can gather, the preputioplasty is mostly done in europe, but most urologists in the states immediately recommend a circumcision.

And why you think most urologists here in the U.S. immediately recommend a circumcision? Yeah, right, because that's the most effective method for curing phimosis!
