I have that clunking, grinding with terrible pain under my kneecaps, both of them.
After xrays and having my knees looked at inside with an endoscope, my orthopedic surgeon told me that I have osteoarthritis on the undersides if my kneecaps and inside my knee joints. He told me that the arthritis on the kneecap is very common in women as they age. I'm 55. While the damage to the cartilage and pitting in the bone is caused by osteoarthritis and not lupus, it is much more painful during a lupus flare because that increases inflammation everywhere in my knees.
The good news is that I had OrthoVisc injected into both knees last November and my knees STILL feel so much better. OrthoVisc and SynVisc are thick materials that stay in the joint capsule to lubricate the cartilage and bone surfaces that have been damaged by arthritis. The doctor told me that it doesn't work for everyone, so I am especially thankful that it worked for me!
My doctor did three injections a week apart into each knee. It is more comfortable to have a smaller amount injected three times rather than a larger amount all at once. Only once did I have a little achiness in one knee for a day after the injection.
The material injected is expensive but my private health insurance covered 90% of the cost here in the US.
Maybe you can ask your rheumatologist if these injections might help with your knee pain. I'm not sure if the materials can be injected into other joints like ankles.
