Do you receive a UK Personal health budget or integrated personal budget?

If so, NHS England are inviting you to share your experience of the care and support you receive in a new survey.

The survey is now now open and will close on 30 April.

All survey responses will be anonymised and used to help improve how personal health budgets are offered in England.
