Could anyone help me with my test results?

My doctor is concerned and wants to refer me right away to consultant. But I feel absolutely fine - nothing like how I felt a couple of years ago when I was last hyper.

I was first diagnosed in 2013 was fine after 1 Year Carbimizole. Relapsed end 2015 and did block and replace - have been off medication for nearly 2 years now.
