COMPLETION OF FORM FOR REVIEW OF PIP

I just wonder if you have any tips regarding completion of PIP form regarding fibromyalgia. I have had PIP for around 4 years and am due a review very shortly and have received the paperwork to complete but feel its a bit of a nightmare as to what to say. When I applied for PIP 4 years ago I had help with the form but do not have that help anymore.

hi, i've just completed my review form and successfully awarded the same rate for another 4 years.

do you have a copy of your original form ... what i did was copy and paste the answers from my last form, and then just amend the answers to show any necessary changes. If there was no significant change from time of award i said so.

I also included 2 diaries ... one is a diary of how the descriptors affect you, the other was a pain/symptom diary. Both diaries were detailed in how the descriptors affect me dailly.

I would suggest completing the information fully on how you are affected - not only the changes since your last award

Hi Highlander50 - As mentioned, you should complete your PIP review with as much detail as your initial claim pack. As you'll see the review form is much shorter, so add extra pages at the end, saying which question the additional info is about. Please make sure you add your name & National Insurance number at the top of each additional page.

The FMA UK PIP guidelines may have these details, but sending the following link just in case, which lists the activities/descriptors that are looked at for PIP, & also reviews. Please see: cambridgeshire.gov.uk/asset...

Also read the notes at the end concerning 'reliability,' i.e. you should be able to do an activity safely, to an acceptable standard, as often as would normally be expected, & in a reasonable time period (no longer than twice the time it would take someone without your disability).

You should give a couple of recent, detailed examples of the difficulty you have for each applicable activity. Why was this difficult, what exactly happened, & when, & what, if any, was the consequence of attempting/doing such an activity? It's these details that are needed. A diary is indeed useful, both for noting to yourself how you're affected, & to send off with your review form. If your condition fluctuates, mention this....PIP is about how you are the majority of days, i.e. how you are 50+% of the time. Good luck.

Hi Highlander50 ,

I recently had to complete my PIP review form. I couldn't face doing it on my own this time and got help from Fightback4Justice look them up on google. They have a form filling service which you have to pay for but I found it was really worth it. They were great, they go through the form with you over the phone and complete it on your behalf.

Hi I also used Fightback4justice to complete my PIP form. It is worth the fee which is money which goes towards the expenses of running their office . Their success rate is exceptional. The founder is a Lawyer. Good luck

I have recently completed the renewal form. I have Fibromyalgia, Rheumatoid Arthritis, Hearing Impairment and have severe Osteoarthritis in both knees. In addition my artificial hip is loosening. I redrafted and completed the form on a word processor. I couldn’t have managed it by writing. I sent copies of e-Mail exchanges between my GP, Surgeon, and MP. Photographs of all the equipment I use in the house. A friend of mine who is also an Occupational Therapist, and has known me 40 years wrote a professional assessment of my current situation.

Much to my surprise I got it back fairly quickly with PIP reawarded for enhanced Care, plus the addition of enhanced Mobility, for 4 years. They didn’t even bother with a telephone assessment. It was a great weight off my mind. Their comment was basically the deterioration was in keeping with my medical history, which was a bit mind blowing, given they usually completely ignore that....!!!
