Go to the website "benefits and work" where they have a guide for all benefits, assessments and appeals. If you sign up to their email they often include a discount code to reduce the annual cost of £19 - it is the best money I've ever spent!
In addition, get in touch with your local DIAL disability charity & an advisor can help you fill the forms in.

I totally agree with BlueMermaid3 as the CAB can help you with filling in your forms and advise what to say / not to say at any assessment.
