hi have been on dla for 7 years and suffer fibromyalgia ive been told to claim for esa i rang up and they told me to get a medical certificate from the dr and they would send me a form out has anybody filled the form in and have you any tips to help thankyou

Hi there if you get an appointment to see your doctor and ask if he would support your claim for esa with a sick note. When filling in the form tell them about how it prevents you doing things and how long if that has changed since you've become ill. Really hopes this helps. doctor dont like filling in forms for this best just go citizen advice get them fill form exsplain how you feel got be on your worst day that what thay go on how effects you pyschal etc that hoe i got support group get an appointamant with cab ok works for every one are trained how fill the forms

with your condition your get in the support group esa that indefinte type thing accept have life long condition thay put me wrag group wr activity group i appealed said not doing any type work won appeal been in suppoty group two years big benifit end not have pay any rent or councill tax which thay were charging me bit off

ESA comes in two forms, contributory ESA based on your National Insurance record which means you need to have worked in recent years, or Income based ESA which is fully means-tested. Contributory ESA is only mans-tested against pension, annuity and income replacement insurance income.

fibromyalgia Action UK has in the past supplied members of this forum with ESA guides produced by Benefits and Work that take you through the forms step by step. you should try asking them for the guide. Citizens Advice can also help you fill in the forms. Th first claim form is easy but the detailed one is called ESA50 and is used by the Disability Healthcare Professional who will meet you to assess your disability. DWPnthen use their report to decide whether you get ESA and if so which group you are put in, the Support Group or the Work Related Activity Group (WRAG). If it is the WRAG you will be required to participate in activity to prepare you to return to work, and ESA would stop after a maximum of 12 months.
