Info@fibroaction.org with the heading of the benefit you are trying to claim or appeal. Emma can send you out the relevant sheets to help fill in the forms for free

hi i would just like to say these guide sheets are worth the world emma emailed me them straight away.it seems a bit mindblowing when you first start to read through them,but they are invaluble,as a total guide for claiming esa,i followed what was advised,took my time,to answer all the questions and how my health effects my life,and photocopied every thing i had to send to atos,appointment letters,doc letters,medication scripts etc,and 2 wks ago i was sent a letter that i had been put straight into the support group,so once again anyone going through this take my advice get the advice sheets as mentioned,take your time,they can only help and many many thanks to emma and all at this site,wishing you all a pain free day x
