Letter to Doc for referral?

Has anyone had to write a letter to their doc explaining their symptoms, who they want to be seen by etc? I am seeing a new doctor at 4 and want to be taken seriously so would like a letter to take with me.

I would absolutely put everything you can down in writing to your GP, maybe with a bullet list of symptoms and history that is easy enough for your GP to see and reiterate in a referral to a consultant. You know everything about what you feel, so give your GP all the info you can about that.

I did but my computer wasn't working so it wasnt a formal letter just written. My new Doc seems ok but I don't think he really knows about Endo. Because of my symptoms he's now put me through for more blood tests, this time for Celiac disease and blood sugar. He hasn't refered ME to the specialist I asked, instead said to do all the tests etc locally and then cross that bridge when we come to it :/
