My serum creatinine is fluctuating

Hi All,

My serum creatinine is fluctuating it was 1.2 one year back then it reduced to 1.0 and then reduced to .85 then it increased to 1.1 now it's 1.2

Do i have CKD?

Protien in urine is 380mg for 24 hours.

what will be my life span, i am not diabetic and don't have High BP.

I am 31 years Male.

Kumar. First, calm yourself. You are getting overly upset on this. You are thinking about life span?? No one has said you have even a problem (a doctor). You are NOT dying. All the numbers you report are NORMAL!! Also, creatinine can fluctuate as much as .2, from one day to the next. On the 380mg of protein in 24 hour collection. Was this 380mg/g or 380mg/dl or??? It is high. However you left out a most important piece of information. That is what you doctor said? He has access to all the areas of your blood and urine tests. To me (not a doctor) the creatinine wouldn't bother me a bit. But I would expect my doctor to comment about the 380mg or protein. However we do not see all your other testing results. And we do not know if you have any other medical problems. Protein in urine can be affected by many things, not just CKD. But you certainly do not need to be thinking of life span. What did your doctor say?

Thanks for your reply.

380mg for 24 hours urine collection(13mg/dl).

Eight months back i visited the doctor couple of times, he told 380mg for 24 hours is slightly above the normal and not need to worry, he told serum creatinine 1.2 is not a good sign keep watch on that and told to come after one year.

other scan reports were normal.

Every month i had test for creatinine, below are the result

Jan 2017 - 1.2mg/dl

Feb - 1.1

March 1.0

April - 1.2

may -1.1

June - 1.1

July - .85

Aug - .85

Sep - .85

Nov - 1.1

Dec - 1.2

Oh my gosh, you are doing fine in all categories. Go enjoy life! 13mg/dl normal. To hit 1.2 a couple of times is far from alarming. For a male even up to 1.4 (of course depending on lab reporting) is considered normal. A creatinine result can be influenced up to .2 points just by the amount of water you drank the day or night before the test. Why on earth your doctor said that you got a 1.2 is a bad sign is surprising. I imagine what he was saying is you were averaging .85. Then trended to 1.1., now 1.2, so he wants to be sure that upward trend does not continue. But you did the same in January and in April, and then drifted back down. No need for alarm bells. Again though you obviously have medical issues, because there would be no need to test your blood so much. So your other problems may be influencing your fluctuations. But from what you have reported, your Creatinine and protein are NORMAL.

Hi RickHow,

Today my serum creatinine is increased to 1.3mg/dl, i am really worried.

in 12 days it increased by .1mg/dl.

Till now my serum creatinine was in normal range, this is the first time it has went above normal.

Kumar: I am a 65 year old male. My creatinine was only JUST called out for being in the 1.2-1.3 g/dL range! I went over old lab tests and found out that it has been 1.1+, 1.2, 1.3 for at least the past 15 years! Since the levels are remaining pretty constant, I am not concerned at all. The fact that they are at the top of the normal scale (and a bit beyond) don't bother me simply because they could mean anything. That isn't much higher than you - and has been that way for a very long time.

If your protein stays within a normal range, then you are fine. Slightly high may be normal for you, or be caused by other issues. I'm sure your Dr. will keep an eye on it, though telling you that 1.2 is not a good sign is really wrong on his part. My Dr's tell me the opposite. Urine protein is a far more reliable indicator of kidney problems than serum creatinine.

May I ask who/how you had blood tests done every month? If you had someone order them, then it was just likely stress talking. If your Dr. ordered them, then it seems excessive. Two months in a row, maybe three - but a whole year? If you have another ailment that triggered that many blood tests - then it is perhaps understandable.

If you are not diabetic and don't have high BP, my advice would be to stay that way. Keep exercising and try to eat reasonable well. In 40 or 50 years you will be the better for it. Good luck.

Today my serum creatinine is 1.3, in last 12 days it has increased by .1mg/dl.

I am really worried. i will be visiting the doctors in next 2 days.

I didn't had control on my diet is that the reason for increased serum creatinine?

You can even go to two labs and get two different results on the same day. Get your creatinine checked in another 2 weeks. If it keeps increasing - go to a Dr. And yes, your diet could have changed it that much. 0.1 change with a prior value of 1.3 is not a large number. If you had increased to 2, THEN they would likely have you repeat the test. Only if it was really around 2 would they check you further. How your creatinine is trending is important. What was creatinine 6 or 12 months ago? 2-3 years ago? Yes, a rapid elevation in creatinine can indicate a problem. But The numbers you give (especially without older values) can just be test variation. These problems can get complex, and a 2 week variation of 0.1 mg/dL in creatinine will not indicate that.

Also, what’s your urine protein? If you are not leaking protein, then even 1.3 serum creatinine means a lot less. There are a lot of aspects to CKD. You’ve got to look at the whole picture. Creatinine is part of the picture. Try to relax - please. These are all CKD screening values. They can vary a lot. When more than one aspect of kidney function changes - then you need to look at it.

This is why I have a big problem with the creatinine-GFR-stage 1,2,3,4,5 system. It is too simplified. It was invented to simplify a complex foeld (nephrology and CKD), but it went too far with simplification. It just creates a lot of stress where stress is not always necessary. I wish when they give these screening criteria that the place a little “*” at the bottom, where “*” means “ not so simple - check other values too”!

Daily walking+ Normal Diet : my creatinine went to .85.

Without Walking + no diet control : my creatinine is increased to 1.28.

Below is my serum creatinine for Month wise:

Jan 2017 - 1.2mg/dl

Feb - 1.1

March 1.0

April - 1.2

may -1.1

June - 1.1

July - .85

Aug - .85

Sep - .85

Nov - 1.1

Dec starting - 1.2

Dec Ending 1.28

Though creatinine was supposedly chosen because it tend not to fluctuate, these results are odd. If you have a fluctuation from 1.2 to 1.1 - then that makes sense. If it went from 1.2-1.05, it will still make sense.

All I can say is where were these labs performed? They trend up and down and back up again - but they are all within a normal range. I've never seen anything like it. It would be hard to say this had anything to do with your kidney s because the creatinine makes no sense. Why did you have it measured so much?

I bet your Dr. is confused and wants someone else to take a look at it. Unless you had huge fluctuations between lifestyle and activity level (and I'm talking HUGE!) I can't even guess what could account for it other than a really bad lab running the samples. Come to think of it, that is my #1 guess - that the lab is a disaster. If you had creatinine measured every 6 months (more typical), it would not show such erratic values. It may be because you are seeing the values of each month - the variation is more obvious.

Some clinics run their own metabolic panels. They are simple to run and on automated machines. An example would be an urgent care clinic. They often want the results ASAP and use their own lab test equipment to run these. Of course these machines are not as accurate as big blood lab machines. I went to an urgent care clinic last year and the metabolic panel they ran was totally out of sync with my "hospital lab" results from around that time. Sorry, but that's all I can think of. Even the July-September results of ".85" repeated three times. Why not a ".89" or some other value somewhere there? 3 ".85" results in a row is suspicious, unless their machine can only to ".8", ".85", ".9", etc. Do you see my point? Then again your last December result was 1.28. Was that run at the same place that the prior 11 measurements were run? It's the only number that isn't "rounded off".

Any insight here? Or were these run at a major blood lab?

I have done this at the local hospital,

I have cross verified with some of the other hospitals and results was same, I have verified, long back I consulted my nephrologist regarding it that time the fluctuations were small but he was not able to answer about my fluctuations.

I am planning to visit my nephrologist in Jan once needs to see what he says.
