Sugar levels flatmates. Insulin dependent

my HbA1c is 7.7. My age is 69. My Fasting Blood Sugar ranges between 90 to 130 znd PP ranges from 135 to 165. My Height is 170 cm and weight 65 Kgs. I think I am doing just OK on Blood sugar front. I am restricting my Carb intake though not following LCHF diet

Dear Sandybrown,

Yes,I am a diabetic for last 31 year . Initially for about 13 years I managed without medicine and by regular exercise (jogging 5-6 Km per day+ walking)and diet control but thereafter could not manage without medicine. Now I am taking Istamet 50/1000 +Amaryl 3mg both two times a day (with Morning breakfast and with dinner).I am continuing 5-6 Km walk everyday. My typical daily diet is morning tea at 7.30 AM with some dryfruits and biscuits. Breakfast at 9 AM 4 times a week 2 eggs + some curd with Oats and two dried figs. Mid morning snack some Papaya.Lunch One chapati+ Dal+ Vegetable Curry+ 5 -6 spoon cooked rice with dal and vegetable+ curd or Butter milk. Evening tea at 5.30 pm with some light snack (home made), Evening 7 some fruit (guava) and dinner one and half chapati + vegetable curry and curd. I never tried to measure calory or Carb intake but I think it is quite reasonable looking to my present level of BS

I thank you for giving me all this information. So far I am managing without any medication, only time will tell!!

We do not measure any carb in our home cooking. I am retired therefore I can go to gem every day. I usually spend 40 minutes in the morning before breakfast. Time between my evening meal and breakfast is 14 + hours. Breakfast and lunch, 3 hours.

I always have dinner before 7PM. So far it is working fine. I go for eye screening, foot check and blood test HbA1c once a year. My main problem is controlling food portions per meal.

Breakfast, porridge, cinnamon powder, turmeric powder, fenugreek powder with milk and honey.

Lunch, brown bread, butter and cheese.

Dinner, it varies, rice and curry twice a week, other days different mix, I always use olive oil with every evening meal.

Your BMI is 22 which is healthy..not to worry...

However on HBA1C, there is a scope for bringing down close to 6...

Diet and exercise should definitely help..

(1) Have you ever checked your blood insulin level or C Peptide levels which would indicate your Pancreas beta cell status?

(2) are you on any medication ??

I never got tested for C Peptide or Blood Insulin level. I will discus with my Endocronologist next time when I visit. I agree that there is scope for reduction but I think there is not much scope for diet reduction and theDoctor says that at my age I have to manage below 8 and preferably go upto 7 if possible. There is risk of Hypoglycemia if I reduce diet or increase medicine. My fasting BS is quite low

There is no need to reduce the food intake in order to reduce blood sugar levels. Diabetics need not remain hungry. What is important is that a diabetic should avoid or minimise the intake of those foods that give high rise blood sugar. Looking at your typical day's menu, there seems to be some scope for improvement without reducing the quantity. Medicines should be adjusted according to our food intake. Below are my thoughts on your diet described above:

1. Morning tea with dry fruits and biscuits:

That is a -BAD START OF THE DAY. Dry fruits are loaded with sugar and biscuits are nothing but mostly carb. You should avoid these.

You can have some nuts with tea. I am sure you do not use sugar in your tea.

2. Breakfast:

Eggs and Curd - VERY GOOD CHOICE.

Oat and dried figs - consider replacing with upma made with almond flour + fresh fruit.

Being vegetarian, there are not much you can do but you can add some nutrient dense items which will keep you full for longer. Try to include paneer, cream, ghee, butter, coconut oil, Olive oil in your diet. These are the simple steps you can take to bring your HbA1c level lower. You do not need to measure calories. Eat until full.

HbA1c 6.3 with no medication - controlled by diet and exercise and have no need to check carbs, I check enough already. Weight 11 stone height 5.7"

Type 1 diabetic very insulin sentitive. All numbers must be accounted for and we must cross check ourselves daily to ensure we are staying within our doctors parmeters. Being a diabetic is not a joke. a 6.3 is excellent keep up the wonderful work. Exercise and a closely flowed diet is important. Take all med's and do blood readings. Rakeeh a 7.7 at your age is also not bad get out and do some walking with some buddies or adopt an older dog and they help get you out and about and keep one out of junk food. That is critical for all on site clear that out of the house, eat clean and eat less. When full stop!

American standards prescribe it and my Doctor also confirmed it. Still I am working on reducing to to below 7 and perhaps my efforts may be successful

My c peptide level is 0.987 nd/ml ..any idea for how long I can go on without insulin injection with this c peptide level? Currently I am on medication

I think last time you said you are already on Gliclazide and Metformin...

Diet with low carb and exercise along with your medication will definitely keep you fit for decades.. keep your Hba1c @6.

There are people like you who manage their condition without exogeneous insulin for decades...☺️

If you could read my recent posts- it has been discussed quite a lot on the topic....

Thin type 2 Diabete may have the following features...

(1) In spite of having Low body weight and low BMI (18-21) the person still keeps losing weight.....and he becomes skinny and bonny..

(2) His fasting insulin consistently remains low as low as 2 to 5 uIU indicating that his pancreas Beta cells not able to produce sufficient insulin...

On the other hand, the Obese type 2 may have very high Insulin levels in the blood persistently ...His diabetic conditions could be due to Strong insulin Resistance ....He would find it difficult to lose weight and sustain it...

Depends on how long he has know and is getting adjusting to to being a diabetic. Did he start higher and is now just reading this or is he working to descrease his numbers. I pointed out some things that might help him start. Changing behavior at 69 is not easy. We have to let him know we are here to support him along the way, and have any groups that are super active he can get involved in to help him. We do not have a complete history.

I agree that at the age of 69 it is not easy to change. Moreover as rightly said by my Doctor at this age he will worry when my HbA1c exceeds 8 and the the American standards also say. Still I am trying to reduce it and working on it

I am 65. Started diabetes in 1984 October. Started with Type 1. Recent, HbA1c is 6.7 Diagnosed as CKD, but no medication to now for the past 10 years. Creatinine is 1.7 for the past 10-15 years. Present situation is mainly flatulence and weight controlled within 63 to 68 kgs. Feel terrific, when the flaulance troubles else feel completely normal. Get a reasonable sleep 7-8 hrs. a day. Sometimes headache troubles.

From what I have read that type 1 person do need insulin, many different type depending up on the condition of type 1. When you say no medication, my question is how are you controlling your type 1 without any insulin?

The last UK prime minister with type , has a patch to monitor blood glucose level and insulin medication to control it!!

Agree, What you said is absolutely correct if the person is an Obese type 2 with very high circulating blood insulin levels. These people have a strong IR, which could lead to inflammation.. so reducing insulin levels and improving insulin sensitivity by all natural means is advised.... reducing weight by Low Carb diet exercise....oral Insulin secretagogue definitely not required... Or else he becomes heavier and heavier...☺️

However, If he is thin type /under weight/ Low BMI with extremely low level of Blood insulin level indicating impairment of beta cells, he badly needs insulin for food metabolism and the solution for him would lie in an oral Insulin secretagogues after consulting his Endochrinologist...this is what all the leading endochrinologists are also advising..though the dosage can be minimised with suitable modification in diet such as low Carb and exercise...

Insulin is a master hormone and helps in metabolism of Carbs, protein and fat...and due to lack of insulin in his blood, he becomes thinner and thinner....☺️

So both excess insulin or lack of it are bad....

And even for an obese type 2 with high level of initial blood insulin levels , if weight is not reduced / IR is not controlled , it can finally leads to gradual impairment of beta cells and at some point of time they would stop producing insulin...

And whether he started as an obese T2 or thin T2, when the betacells totally stop producing insulin , one has to resort to exogeneous insulin for survival ......thank God for the discovery of Exogeneous Insulin...

Seems to me diet is saying that some medication is taken. Diagonosed in 1964 so some years are are missing. Good question though sandy I'm Type 1 and wear a DEXCOM monitor 24/7 that monitors my blood sugat levels. Ranges are set mine 80-200and a alarm will sound on either end. I run very much on the hypo end can drop into the 40's in a heart beat. Sometimes this is why these individuals will have low readings. But the person feels great and that is wonderful. I'm getting old and it's wearing on me besides seizures. I can go very closely monitored with out insulin for a meal or 2 dare not push the hight need my long acting and short acting.

There are some ways of controlling eat a Keto diet, I'm thin and my endo and neuto want my weight higher so to fighy the carb. unless I consume a special pasta-Dreamfield. I perfare my say Shrimp Risotto minus the shrimp and freeze the base will reduce carb. level. Have experimented on many carbs., works great. Keeps the carb level down allow without alot of ready one would not stumble on this fact. Excise is one of the best over all motivations. I do not hear it on our the diebetic site but so many people complain and whine they can not do it to many kids 2-3. Do it with them get them involved get then a tiny broom and mop. Have them get on the floor and rock and roll to a record or TV with exercise. Tell them to get mom in shape or dad! The group is here!
