B1 Therapy- transitioning from oral to sublingual

Hello PWP Community,

Does anyone have experience in converting from oral dosages to sublingual B1? I’ve been having very good experience with the Oral HCL of 2400mg daily in 3 x 800 or 4 x 600mg. I’d like to convert to the sublingual as its purportedly a smaller dosage and single administration daily. I have been taking orally the Solgar 500 tablets with solgar 100 capsules both HCL. I recently discovered Dr. Clark Vitamin B1 HCL in 500 mg that claims to be free of Magnesium Sterate which “hinders absorption”. I since discovered that the other B1 products generally do have the MS! Is this claim true? Does Magnesium Sterate hinder absorption and is this why the oral doses have to be so high?

Do people have a routine where they use both sublingual and some oral supplementation? Or does a single dose of sublingual 100mg suffice?

Thank you in advance for your consideration and kindness in sharing your experience and in general for all your thoughtful contributions in this forum🙏

I would not do that! Never touch something that works well... Guess you know this proverb..

No any guaranties that you will achieve the same effect. B1 dosing is a very sensitive individual matter and you may end up in a deadlock. Do not change, if no necessity to do that..

Agree, do not change something that works and no guarantee that you will find correct dose of sublingual. At least, wait till you have months of experiencing the right dose of HCL.

First, magnesium stearate can reduce the absorption of certain nutrients. This is because the compound forms a barrier around the nutrients, making it difficult for the body to absorb them. This can be a problem if you're taking supplements that contain nutrients that are essential for your health.Aug 21, 2022

This article has in depth information on magnesium stearate.

Hello Boscoejean. Thank you for responding to my question about Magnesium Stearate. I wonder how significant the mal absorption is? I’m going to switch from the Solgar 500 mg B1 HCL Tablet with Magnesium Stearate to the Dr Clark 500 mg B1 HCL capsules without MS. I suppose the capsule will also be easier to digest? I appreciate your attention and the information you highlighted.

yes I wonder too since I just found this by doing research not personal experience

I was on 1000-1500 mg B1 HCL Vitacost capsules and changed to sublingual. I'm going back to capsules for 2 reasons. First my bowels seem much more regulated with the capsules (i used to be regular and got some constipation when changed) and because i find hard to keep a pill under the tonge for half an hour without swallow, so i never know the acurate dosing.

Thank you Markbit for sharing your experience. Can please I ask a few more questions? How long were you taking the oral capsules? For how long did you try the sublingual B1? What dosage did you take sublingually? Did you find the sublingual effective other than the constipation? Your consideration and kindness is appreciated.

Hi Nico, i take the oral capsules from 4 years now. I did never saw any imediate improvement, but i believe it has been one of the things that is helping me to control de disease. i tried different doses, from 500mg up to 3 g, but at that dose it caused extreme dizziness. Never really found a sweet spot, but i'm sure it's helping on the long run (i'm 49, first symptoms at 34, dx at 35). I changed to sublingual 2 months ago, 50 mg/day, and didn't notice any diference, perhaps a little increase in symptoms.

Thank you for your reply. There are so many variables I know its frustrating trying to find that sweet spot. God Speed!

Markbit,

I would be very interested in any and all details you can share about your total B1 experience such as how you did on thiamine HCL and at what dose compared to what you are seeing with Superior Source melt in your mouth tablets at 100 mg/day and 50 mg/day. Thank You.

Hi Art,

I have been using B1 for about 4 years now. I always tried to keep the dose as much high as i could stand, and tried, mainly because Roy, who seemed to be one of the persons with more experience around here with B1 was always pushing for an high dose, that the therapeutical dose was 4 grams, so i keep a dose betwen 500 mg and 3 grams, with several variations along the years trying to find my sweet spot, I even tried once 4 grams, but felt so dizziness that only stayed for one day. A few weeks ago i saw a post on Daphne's facebook group saying that usually a person see some improvements, and when it get's worse, tends to up meds, and does the same with B1, The thinking is, my symptoms are getting worse, so the disease is progressing, i have to increase meds to aleviatte symptoms, when with B1 it's the opposite. I saw some comments of people saying that were getting great results with sublingual, and i decided to try myself. Latelly i was taking 2 grams B1 HCL, and switched to 50 mg SL about 3 months ago. Stayed on that for about 6 weeks, but i was not getting any better, and was finding hard to keep the pill under the tongue without swalowing (i have lots of saliva), so went back to B1 HCL 1,5 g. Meanwhile, i started wondering if i have not been overdosing all this time. The improvement i can really link to B1 is related to solving constipation, wich have been worse latelly (and as a mosquito repelent), So i stoped for 2 weeks and i'm restarting now, was thinking on 500 mg HCL, but i decided to give sublingual another try, So, started yesterday at 50 mg sublingual, and plan to stay on it every other day for 6 weeks and see if there are improvements before increasing any further. Hope it helps, if you want to know anything more especific just ask, glad to help, sorry for my english.

Marco,

Thank you for your reply to my inquiry. Your English is very good!

I never understood why Roy always pushed 4 grams as much as he did, because that was not what Dr. Costantini was telling this forum at all. I have always made it a point to mention that the effective dose range reported on this forum is 25 mg/day up to around 4 or 5 grams/day. I have only seen one person mention the 5 gram dose, with one person mentioning 6 grams and a few mentioned the 25 mg dose. The majority of people find effective dosing in between those two doses.

The dissolvable pills that Superior Source sells are not a special sublingual formulation, but rather, the thiamine mononitrate (TM) itself is sublingually absorbable. They use their trademarked term, "Microlingual" to describe their pills. I assume they do this to imply that their pills are a special sublingual formulation, but when you look at the other ingredients, you can see they are counting on the fact that thiamine mononitrate by itself is sublingually absorbed. TM powder by itself is likely to be as effective.

If you are mainly taking the thiamine to help alleviate constipation, I would not think the Superior Source pills would be as effective as thiamine HCL high dose capsules from Vitacost, because if the thiamine mononitrate is actually being sublingually absorbed, the thiamine would mainly be bypassing the gastrointestinal tract, the area where it would likely be helping with constipation.

So the only benefit you get from thiamine at any dose is relief of constipation?

Hi again Art

I do not think that constipation is the only thing that thiamine helped, but is the symptom that i can clearly associate with the intake of thiamine that had a notorious change. even got some dhiarreia at the beginning. I believe that thiamine really helps, thats why i never stopped taken it. But i also do a lot of other things, like exercise, diet, other supplements and a social higiene, so it's not easy to tell the roll of B1 in the middle of all this, and also did an PTT FUS almost two years ago, wich has to be highly considered. I think all of this, which i was lucky to be aware in a more early stage of the disease, has helped to manage the disease. I'm doing 50 years next year, got the first symptoms 15 years ago. but i'm still working at school , still driving, i set up 3 new businesses in the last couple of years, so i'm not in bad shape, at least compared with some others around here. This mornig i got up and went for a 18 km/ 10 miles bike ride. Back to thiamine, i was thinking to go back to HCL, but decided to give one more try with SL, as i have seen some comments that people changed and have noticed noticeable improvements. My worst symptoms are now some tremors on my untreated side (arm and leg) and walking dificulty, wich i think is becoming responsible for my lower back pain, as i drag my right foot in some hours of the day, and i'm hopping that maybe thiamine will help a bit more if i can find a more effective dosage.

Thank you for the reply, Marco.

I see with all that you are taking, it would be impossible to discern what B1 is doing. I am though, very glad to hear that you are doing as well as you are after so long with PD. You must be doing some things that are really helping you!

If you haven't already read the following link, you may find it helpful in zeroing in on your optimal dose of B1. I wrote this over 2 years ago to try and help people who are just starting HDT :

Hi Nicopwp

It took me around a month to find the right dosage. I started 100mg a day but it was quite high dose and started having worsening of symptoms. I had a short break and then resumed with taking 100mg for 5 days a week. Again that also wasn’t the right dosage. I finally got it right with taking 100 mg Microlingual B1 every other day. I feel sharper and more energetic than taking orally but other benefits remained the same. Hope these help. Kia
