Parkinson's Disease: emerging treatment target, the NLRP inflammasome.

Hyperstimulated immune cells in the brain are emerging as a hallmark feature of most neurodegenerative disorders—and Parkinson’s disease is no exception. In patients with the progressive movement disorder, those immune cells, called microglia, react to the presence of α-synuclein-containing protein clumps by dialing up inflammatory signals that not only contribute to the gradual death of dopamine-producing neurons but also recruit more pathological proteins, creating a vicious cycle of neuroinflammation and α-synuclein buildup in the brain.

Preliminary analysis from the CAPS part of this study showed that a patient with a confirmed NLRP3 mutation suffering from a CAPS-related flare showed rapid clinical improvement within hours and remission within days. A follow-on Phase II trial is planned to identify the best dose for CAPS patients.

It's very encouraging, although I have been cautiously optimistic about it for four years. I sure wish trials could be done more quickly, but I suppose we don't want corners cut either. Thanks for sharing.

Among many of melatonin's activities against PD, inhibition of Inflammasome is one. This is also one of its methods of action against Covid-19.

What can we do NOW? (I like to use current trials to determine if there is anything we can put in to action NOW. While we wait for this NLRP inhibitor to be developed, is there any other means of inhibiting the NLRP inflammasome?

Beta Hydroxy Butyrate and keto were both mentioned in this paper.

There are others that have shown the ability to inhibit inflammasome to varying degrees. Another popular one would be NAC, but it takes more than just that for PD and Covid-19. A very basic example would be the fact that melatonin has saved lives in Covid-19, but NAC has not.

I take NAC, in my opinion, it (along with quercetin & myricetin )saved my life from my recent COVID infection.

I don't doubt it.

Dr. Neel has had similar results with melatonin in over 2000 of his Covid-19 patients and melatonin is very inexpensive. A viable treatment option in many countries where access to frontline treatment options are unavailable or for people who don't have insurance. Now that Ivermectin pricing is going up, melatonin is still inexpensive and readily available to the masses at their local drugstores. Melatonin also comes with the potential for other health benefits and also protects the vasculature and endothelium in particular against the damage that Covid-19 has caused in people who ended up having a stroke or heart attack. The anti-Covid effects of melatonin are pretty much ignored by the FDA who seem to much prefer the current standard of care which is very costly. Melatonin has also shown benefit to people already on a ventilator in terms of increased survival rate. Melatonin checks many of the boxes for what an ideal Covid-19 drug should do and in PwP studies it helps against PD also.
