immunohistochemistry need help understanding

I am seeing my doctor on Tuesday but was able to get some results online from my bone marrow biopsy - Additional tests ordered: Cytogenetics and FISH (CLL panel)

Can someone help me understand the findings below - thank you

I am not an expert. It appears to me these are the results of a flow cytometry test which looks for molecules on the surface of cells indicative for Cll. The test means you have Cll, which you know.

The negative TP 53 refers to a cancer fighting gene that is sometimes damaged with more aggressive types of Cll. I would assume the test being negative is a good thing, your gene is intact, but ask your doctor on that.

You seem interested in knowing more about Cll. I see it as a know your enemy thing, which is why I have tried to know Cll to know better how to fight it.

Hello kp, I've had a chance to look at these results, and have written my comments in ( ) next to each. A first question though is have you been diagnosed with B cell CLL? Have you had any flow cytometry that looked for CD19? The reason I'm asking is that it unusual for CD3, a T cell marker, to show up on a CD19+ B cell.

Your results - (My layperson comments)

CD20 Positive - Pan B Cell Antigen (L26) (This usually identifies B cells, but CD20 can also be expressed on T cells. Usually B cells are identified as CD19+ cells)

CD3 T-cells Positive - Pan T Cell, Epsilon Sub-Unit of the CD3 T Cell Receptor Complex (This is unusual. CD3 is usually just on T Cells. However, if blood is stored for a while before analysis, the CD3 can show up on your CLL B cells) journals.plos.org/plosone/a...

CD5(4C7) Positive - Pan T Cell Antigen, Mature B Cell Subset, Thymic Carcinoma (The presence of CD5 on CD19 cells usually identifies the CLL clone, as CD5 is usually limited to T cells)

CD23 Few Cells Positive - Low Affinity IgE Receptor, Mature B Cells, CLL/SLL (This is typical for CLL/SLL and rules out mantle cell lymphoma, but can also be expressed on some T cells)

CYCLIN D1 Negative - Mantle Cell Lymphoma (Cyclin D1, PRAD-1) (This rules out mantle cell lymphoma)

p53 Negative - Mutated Tumor Suppressor Gene (PAb1801) (This means you have an intact p53 which is good as cajunjeff said above.)

MUM1 Rare Plasma Cells Positive - Plasma Cells Post Germinal Center B-Cells, Activated T-Cells (The presence of MUM1 in the CLL cells is a good thing! “Our results suggest that B-CLL with expression of MUM1/IRF4, indicative of postgerminal center origin, has a more favorable clinical course and that MUM1/IRF4 is an important prognostic marker in B-CLL.” ) pubmed.ncbi.nlm.nih.gov/123...

Please let us know what your doctor has to say. Especially, please ask about the presence of CD20 and CD3 on the same cell.

Much better! This is 100% consistent with a CLL diagnosis! Thanks. I wonder why the other report indicated the presence of CD3?
