Thanks for sharing! Good report. Similar to the Ultra V with a 3 drug trial and very promising early uMRD results (though UltraV has a Pl3K inhibitor instead of BTK inhibitor).

One different research aspect of the two studies that intrigues me is stopping treatment at uMRD. This BOVen study does so, where Ultra V continues treatment for 24 cycles regardless of uMRD status. Def going to be some interesting data to come out in the next 5+ years from these and other studies about the consequences of that types of decisions.

This is great news.

I think it's probably just a question of which BTKi gives the most preferable side effect profile for patients as this combination is definitely offering the most hope of a treatment free period and a long sustainable remissions. Dare we even say cure for some patients? :)

A durable remission for sure and zanubrutnib seems to less toxic than ibruntnib and acalabrutnib. Great to see science changing the CLL treatment regimen

Hope these triplets get fast tracked for standard treatment

That’s fantastic news , ZOV has some outstanding results. Similar to AOV and ultra V

Apparently zanubrutnib is even better than ibruntnib and Acalabrutnib, less side effects and better targeted.

Now to get these approved from trials to front line therapy

The Ultra V trial doesn’t have many restrictions they are taking relapsed as well . Sllxolorado I believe is starting ultra V This week

Not sure about this trial mentioned in the link above but the Ultra V trial is Phase II and the Phase I version was actually only for relapsed. They all did really well. U2-Ven was the name and most if not all achieved uMRD. Of course the $64,000 question is how long will it last...

My husband is 2 weeks in on Ultra V - was treatment naive. So far so good.

I happen to be on the BOVen trial. I’m starting Cycle 11 having achieved peripheral blood UMRD in Cycle 9 but not in my bone marrow. About 15pct have such discordant results. I get retested in two months and hopefully I have UMRD both in the blood and in the BMB.

Ramp up on O and V were no problem for me. On going tolerance for me has been generally good with side effects such as low platelets, bruising and some fatigue. Recently I developed an apparent “chemo” induced diarrhea resulting in a step down in drugs but the diarrhea is tolerable at current levels.

This is an exciting trial especially when the end point is time to UMRD versus “remission”.

That is great! So hopefully you are looking at stopping treatment soon and can get rid of the side effects.

I'm curious how they sequenced the 3 drugs? What started on which cycle?

Looks like the BO started on C1D1. When did the V start?

Started on the Zanubrutinib at 160mg ibid and the infusions of obintuzinab on day 1 of Cycle 1, and I managed to tolerate the latter quite well and was put on the fast infusions quite quickly. The Venetoclax came in Cycle 3 with the 5 week ramp-up, again no major issues.

Thanks for the info. Very similar to Ultra V with V in Cycle 4 instead of 3, but still 5 week ramp up.. That might help explain how so many got to uMRD so quick in BOVen. My husband has done very well with it so far too. His next infusion will be C2D1 in 2 weeks. We're hopeful he does as well with the V ramp up.

So glad you have avoided the nuetropenia in these crazy times!

Hope all goes well with the V ramp up. Make sure he drinks a lot of water especially each ramp up.

I had no major problems with the zanubrutinib outside of easy brusing (my platlets were low to start with<100) and some fatigue. It’s hard to separate out the other side effects with the other two drugs in place.

True how about the addition of Venetoclax?

I had no real issues with Venetoclax, ramp up or on-going except for the current diarrhea which may be from zanubrutinib in the combo.
