This document sets out how we will all be given greater access to information about our health. It is hoped that this will support patients to play a greater part in their own health planning and know where to get the best care. Health and social care professionals will be able to access patient information that supports them to make services better

CHANGE produced the easy read version so that people who find reading hard have a chance to know what this will mean for them. We also hope that more will be done to make sure that all key health information is provided in a way that people with learning disabilities can access and understand.

Without this people with learning disabilities are at risk of being left behind in the information revolution. Good accessible information supports more people to have power over thier own health care.
