I have been advised 28 session of radiation(Tomotherapy) post 16 cycle of chemotherapy. Till now 12 session done. But due to some maintenance issue, till now 5 days gap has come. I want to know if it impacts the effectiveness of the treatment? I underwent mastectomy and reconstruction of breast in November. I am ER PR positive and her2 negative. Ki 67 - 60percent.

Breast Cancer Radiotherapy is Category 2 where the impact of Gap is small, with slightly increase chance of Locoregional Relapse.
Ideally Radiation Oncologist can try and compensate the Gap by various Strategies like
1) Giving Treatment on every Saturday
2) Giving Radiation twice a day...on few days like every Monday or on/Thursday
3) Increase the dose per fraction to compensate for the Gap (5 days)...Radiation Oncologist has to calculate the lost dose, the possible repopulation and then they complex formula get the dose per fraction
4) Last Strategy...which is not ideal but can be done... increase the Session of Radiation.
Sir down with your Radiation Oncologist and politely figure out which strategy suits both.
Thank you so much doctor for the response. Saturday, sunday was planned gap. And then two days gap was unplanned. Just few hours back I have received the call from hospital that now machine is working and i am on my way to hospital for next session. I am going to discuss the same with doctor how they are planning to compensate. Your answer has put me at ease.
I hope there won't be major impact and can be easily compensated.
