Feedback wanted on COPD Patient Passport

“Do you live with chronic obstructive pulmonary disease (COPD)? If so, we need your help!

We’re after feedback on our COPD patient passport. This quick checklist checks if you’re getting the COPD care you’re entitled to. At the end of the checklist, the passport generates a personalised report for you to discuss with your health care professional.

If you’re interested in taking part, please:

1.Complete the COPD patient passport at: passport.blf.org.uk/

2.Send any thoughts or comments you have on the passport to Alex at astaff@auk-blf.org.uk – nb: please do not send your personalised report or any information about your answers, as this is confidential information

What we’re specifically interested to know is:

•Was the passport easy to complete?

•Were any of the questions confusing or difficult to answer?

•Did you think any questions were missing?

•Would you use the passport report to start a conversation with your doctor or nurse?

We’d be very grateful for any feedback to be sent across by next Wednesday 19 January. Any feedback received after this date we may not be able to take into consideration.

Hi Katie i found the passport easy to understand and fill in. It was helpful in highlighting the areas that need guidance/help or further information.
