Iv sedation and copd

Hi everyone. I am a new member to this forum after recently being diagnosed with moderate upper lobe emphysema and traction bronchiectasis with early honeycomb change. I am due to have 3 teeth implants with sinus lifts, which will take about 2 hours. I have opted for iv sedation and local anaesthesia. I'm very worried that I may have problems due to my condition. Is there anyone who has had iv sedation with lung disease; or does anyone know of someone who has had iv sedation with lung disease? I would be most grateful for any feed back, as I'm stressed out with worry.

Angelapantus,Local anaesthesia is not normally a problem with lung diseases but i would take advice from your respiratory team/consultant re sedation.A moderate diagnosis would not normally preclude sedation but when like myself your prognosis is very severe then sedation is generally either frowned upon or completely ruled out of the equation.Welcome to the forum..............Skis and Scruffy cat x

Hi and many thanks for your much appreciated and prompt reply. My pulmonolagy consultant said it would be ok for me to have sedation, but since reading up on it on internet sites, it's scared me stiff. I should really stop worrying like this but just can't help it.

No problem,go with your consultants advise and try to avoid reading too much on the internet as there is a lot of uninformed and misguided advise on there, good luck with your dental treatment and worry not,,,,,,,,,,,,,,,Skis x

Thank you Skis and scruffy cat. I feel much better after reading your most welcomed advice. Can I ask; was your lung problem caused by smoking? Mine was, and the bronchiectasis from having pneumonia and whooping cough as a child. I have stopped smoking for good now. It's taken the shock of my present diagnosis to make me stop, although I am vaping instead.

Many thanks for welcoming me.x

Hi,yes i believe my emphysema was caused by smoking and like many people of my generation(i am 64)i have smoked since early adolescence,my initial diagnosis was approx 15 yrs ago as mild copd after a diving medical examination,i was advised i would develop emphysema if i continued to smoke which i stupidly chose to ignore and as a consequence i now have very severe emphysema.Well done you for stopping smoking and the best of luck with your vaping alternative.although you have been shocked by your diagnosis you now have the opportunity to take control over your own destiny and there is a wealth of information available to help you slow its progression and live a very long life and i wish you the best of luck in that endeavour.............best wishes Skis and Scruffy x

Hi Skis and Scruffy. Yes I too am from that generation. I'm 69 now and started smoking aged 12. I'm so sorry to learn that yours has now become severe from carrying on smoking after your initial diagnosis but there wasn't e cigs then. If it wasn't for the invention of e cigs, I would still be smoking tobacco as I had tried many times in the past to stop. I've been vaping for a month now and I've completely stopped coughing and bringing up phlegm; also, have had no need to use my inhaler. I think though, had I not been diagnosed with emphysema, I probably would still be smoking. I do so hope that you are managing the best you can and have a good team who are making sure your getting the best treatment possible, and eating healthily and doing gentle exercise, as I believe this helps. Take care and all the best. Angie x

Hi there.

I have had iv anaesthesia twice in the last 18 months once for gastroscopy/colonoscopy and once for a double hernia operation, which was actually caused by the cough I had with my COPD:) . There were no complications either time but as Skis says the anaesthesiologist will advise you and take care of everything.

I did have a problem with the hernia op, just as they were rolls g me into the theatre I had a tight chest my blood pressure dropped to 80/40 and pulse went to 140. Stupid me had an anxiety attack over the ok, but absolutely nothing to do with the COPD.

Good luck with the teeth - I have mine done in Ukraine - a lot cheaper than in the uk:)

Hi and many thanks for your reply. Problem is I don't want to tell the anaesthetist in case he refuses to give me iv sedation. I couldn't go through it whilst aware of anything. Yes I know it is heaps cheaper to have dental implants abroad, but it's a long way to go if I had any problems. I've been saving up for 5 years to get the implants. Do you think it will make any difference to the procedure if I don't tell them about my emphysema?

Sedation with our conditions has some differences to normal. But I would rather them not have any surprises so better to let them know. I doubt they would cancel just maybe have a little extra care. Speak to your GP to get a better view of things

You absolutely need to tell him you have emphysema as it may make a difference as to how they monitor you. Things that may not be a concern for someone that has no lung conditions maybe for someone that has. I really don’t think it’ll make any difference as far as whether they will do the operation, as I’m sure they will still do the operation, just in the way that they monitor you.

Just wondering - even for a colonoscopy where I'll be out for 10 minutes I should ensure the anaesthetist is fully informed about lung condition, then? Or get doctor to send information?

Yes, they should be made aware. I think that's advisable. xx

If your anaesthetist is acting in a professional manner he will have already made you go through a pre med procedure, you need to disclose your background medical history as it will help him/her to deliver the correct amount of sedation in the safest manner,if you advise him that your consultant has already suggested that he does not preclude you having sedation then their is no reason to believe he will refuse you, also it is in your interests to do so.please stop worrying and making yourself anxious.........regards again Skis x

Skis is right Hidden . Please don't try to hide anything from your anaesthetist. He needs to know your full medical history so he can calculate how much medication you need. I expect he will want to see your medical records anyway, which will include the information about emphysema.

Please try not to worry. I'm sure your medics will take all the care necessary. I hope all goes well with your procedure.

Best wishes, Annienell

I have an implant post that has to be removed. My dentist referred me to an oral surgeon who will only remove with sedation. Fortunately, in the interim between call for appointment, during which I learned of sedation only policy, and actual appointment, I was scheduled for my regular gp visit. When I mentioned to him that I was going to have the implant post removed, he said that I could not be put to sleep, because of my oxygen drops. I will have to have a local. Best speak with your gp. He/she might have a different idea....

Hi jmsutt73, just being nosy as I am considering having another couple of implants done as to why you are having the post removed ? I am afraid of getting implantitus and really try to make sure I floss and clean with a water flosser every day. Irene x

The root into which it was implanted is not healthy as it once was. I became vitamin D depleted, and it took a toll on my dental health. I never miss an appointment. It just happens when one becomes D deleted, along with a host of other problems.😜

I have moderate COPD and I had two tooth implants with an IV. Was under for about one hour 15 minutes with no problems. But you should really do this with your doctor's approval.

Hi, I have Broncheichetis and had op last year I had to go to pre op where lots of questions were asked also anehethist came and I had to declare all medical problems, I was told the anaesthetic would be tailored to cope with my condition so I think you would be safer to let medical team know all about you .

Good luck and I am sure you don't have to worry.

I have COPD caused by, TB as a child, Tear Gas, Smoke inhalation in Kuwait and oil inhalation while swimming in the Gulf, and yes I did smoke. Plus slightly overweight but life goes on and at 72+ years have started a new company selling skin care so everybody who has a dream should go for it and not just sit in front of the TV and say "Woh is me". Live the dream, don't just worry, we all are born and as a consequence we all die.

Hi angelpantus, I do think you should let them know regarding your condition, it will help them make informed decisions. I had three implants and a sinus lift with a local and it wasnt too bad, so dont be afraid, its not too bad either way. This was last year before I was diagnosed with bronchiectasis when my daughter lived in Tenerife, it is cheaper over there, am seeing a new dentist here next week so fingers crossed. Good luck and try not to worry. Irene x

I had surgery in July 2017. At the time I was unaware that I had COPD. I didnt have any problems however I definitely would not have surgery without the anacietist being aware in the future. 😊

I had 4 dental implants in two separate IV sedation operations and all was fine for me. I have moderate COPD so am not sure how that differs from what you have. I also had a general anesthetic a few years ago and that went fine as well except I felt like I had been hit by a truck for a couple of days after while the IV sedation really had no after effects except for sleeping for most of the rest of the day.

My gp said no anesthetic because my oxygen drops, so far with no good explanation for why. My guess is my age causes them to be very conservative, particularly with anesthetic. That with the fact that so far they are not having a lot of luck getting a diagnosis.
