Af and high Troponin levels

After suffering with AF for 4 days last week, I decided that I must go to A&E. (really did not want to). Went in at 10.30. Quickly had blood test, ecg, bp etc. Waited an hour and the nurse said I need another blood test. ECG etc were all ok. No mention made of AF or monitoring done. 2 hours later after 2 further ECGs, bp etc, told I needed another blood test. I asked what was going on and why no monitoring of AF. Told my Troponin level was very high 28 and they didn’t know why. After 3rd bloods it dropped to 25 and I was ushered out of A&E without any info and told to speak to my Dr. Feeling very confused and waiting to get a phone call from surgery. Anyone got any ideas. A year ago had angiogram which was completely clear. Do have asthma but no other problems.

Am on flecainide 50mg x 2 and bisoprolol 2.5 x1. Apixaban 2.5mg x 2

Any suggestions greatly appreciated

Sorry nothing specific as I’ve limited experience of your topics - but if I were in your shoes, I’d phone 111.

I think Troponin is a sign that there has been damage to your heart muscle? Can come from a heart attack. Or mine was a bit raised with pericarditis (inflammation of sack around heart). But mine was only 4 & deemed unimportant.

If you do have damage to the heart, I’d imagine it needs investigation & probably treatment. Maybe as the level dropped slightly, they thought it was improving, but I’d still want to speak to someone if it were me.

I don’t know the implications of AF but I know it’s no joke.

Do you have a cardiology contact, maybe somewhere you’ve been seen for AF previously? Maybe call their secretary & ask for ECG’s to be sent?

If you don’t mind me asking, how did you know you were in AF? I had it once (in hospital after heart surgery) and only knew it due to tachycardia (HR 150 for 36 hrs, it was exhausting). Is it possible to know it’s AF somehow? Sorry for questioning back!

I hope you get some better info soon & have a good outcome. I really would try 111 quick, or a cardio doc.

PS is the bisoporol for arrhythmia? Do you have any guidance from docs, to take more if you experience symptoms? I think my mother (had AF) had that guidance once, but bisop didn’t agree with her so she avoided it.

Hi teal green you are correct that Troponin usually shows damage to heart from a heart attack, but my 3 ECGs did not show any signs of this being the case. Nor were there any other indications of a HA. That’s why they just sent me home.

Flecainide can be used as a pip, but I felt my AF was going from high to quite low, and didn’t want it to drop lower. I desperately wanted some advice on my meds but it was not forthcoming. It was a very strange outcome that I am still trying to solve. Thanks for your comments.

I've read about how difficult it can be to diagnose a heart attack in some instances, including ECG etc. Do you yourself feel that was a possibility?

I agree it's a really strange (& unhelpful) outcome, maybe they are avoiding keeping people in A&E due to the virus risk. Assuming you are in the UK, I'd really recommend 111. Several times they helped my mum, often a doctor would phone back & discuss meds to a fair degree, then give her specific things to discuss with the GP. For me, I find I get the best sense from the cardio hospital who treated me last year. On one occasion I asked the pharmacist, but that was mostly about meds interactions.

Maybe a telephone GP appointment could get you a referral, for a telephone cardio appointment to talk through the meds? Sorry I couldn't help more. Good luck.

Hi Potatohead. Following my HA 2 years ago, my troponin level now wavers around in the mid 20s. It worries the heck out of a&e docs until they call a cardiologist down who tells them it's fine! They do have to do the tests though, just to be sure. xx

My husband was repeatedly informed, despite symptoms, that he had not had a heart attack. At an appointment with a Cardiologist, I insisted that he had further tests; this insistence was not taken kindly by the Cardiologist who appeared to think he was highly superior to the both of us.

From what I can remember, the tests included his "riding" a cycle type machine and an echogram, (unsure of spelling). Both these showed that he had had a heart attack.

So, whilst I am not qualified to give any medical advice I strongly advise you insist on these tests if they have not already been carried out.

Thankyou Hullensian for your comments. I'm definitely chasing this up, as best as one can at the moment. I was considered to be very low risk of HA, after lots of tests about a year ago. After speaking to A&E re results, it was suggested that a low GFR could also cause troponin levels to increase, so I am hoping to follow that up as well, as my levels were a bit low in February.

I just had similar and my trop level was 800. They were convinced I'd had a heart attack until I went for the Angiogram and they saw no issues and diagnosed me as AF.

i had my HA my trop was 3000 lol and then 3 hours later 8000 shocked to say the least and im only 47 had stents fitted
