Asthma Control Questionnaires for young children

I am starting a research project in paediatric asthma and am trying to find a (validated) questionnaire to use to monitor asthma control in young children. So far all of the questionnaires I have managed to find are only validated for children over the age of 5. Does anyone know of anything that could be of use?

have you tried your local respiratory medicine unit at a hospital near you or maybe AnE as they are usually the first point of call .
