remicade and arava still have joint pain

I am on remicade 400mg. infusion every 6 weeks ,10mg. arava,7mg.prednisone,seems like it works good until I get to the 4th week,pain and swelling in hands and feet,etc,hands are deforming,and.toes are curling on rt. foot,feels so good after the infusion,they went up from 200mg.as it would only last 2 weeks to 400mg. is this normal,I do work hard with my hands, they cannot go up any more as I am 155lbs. is this from working full time,they raised arava to 20mg. until my blood pressure went thru the roof,drop to 10mg. any answers would be gratefull.....
