Apicoectomy

hi

In permanent AF, take warfarin/aspirin and betablocker. INR range 2-3, normally around 2.2

Need an apicoectomy on front tooth.

Dentist is suggesting stopping warfarin 2 days before procedure.

What are people s thoughts of doing this and is it really necessary to stop warfarin.

My dentist doesn't bother and has done extractions and all sort on me so long as INR less than 3 . Two days isn't really long enough to clear your system anyway so what is his point.

My dentist told me as long as your INR is not too high it's ok otherwise it's a hospital extraction

I have to have a wisdom tooth extracted in a couple of weeks. This will be under local in hospital and I have been told not to stop Rivaroxaban - they said they did not want to increase my risk of stroke. I will probably have a stitch inserted. Has your EP said what he/she would recommend regarding stopping your Warfarin?

My dentist is happy to do dental work as long as my INR is under 3

Just had a tooth out. My dentist told me that most dentists will be happy as long as the INR is under 3. He was happy if it was under 4. He told me they have cellulose pads with clotting factors to put in the gap which disperse in 24hours. I have had my INR tested since and it is still about 2.5.

thanks for all replies.

there is a difference between and extraction and Apicoectomy.

I believe that when a tooth is extracted the blood flow is small. The Apicoectomy is necessary because a large area of infection above a front top tooth cannot be controlled, the tooth has been Root filled but obv something has gone wrong in the past. The apicoectomy is a micro surgical procedure, where a flap of gum is opened up to the jaw bone , thus revealing the tip of the tooth root, and all being well the infection is the removed and treated and then the tip of the root is removed and then sealed back up, Finally 2 sutures are used to close the gum back up.

So its clear there is more hacking than in a straight extraction, yes I have had teeth taken out and root canal treatment in the past without stopping warfarin at all, with out problem, just felt this is liable to open up more blood vessels.
