Should I seek radiation treatment to my spine will undergoing ADT. There is pain.

I started Lupron about a month ago and will start Zytiga next week so maybe I’m just being impatient but, man I’m having very real dull pain in my T2 vertebrae where the cancer lit up. The Radiologist wants to zap it but there is disagreement among the ranks that say in time the cancer on the spine will “melt” away”.

The Mets at the prostate bed isn’t treatable with radiation or surgery. My concern right now is the integrity of the spine.

Thoughts?

give the lupron some time. It could be already shrinking the met causing noticeable different sensation. I would wait at least 4 months

once your full ADT gets crank’in, your pains / discomforts should mostly disappear ( the ones that remain need a closer look ) …. If you can tolerate the wait , it’s good …. If you have pains - discomforts, there is no reason to not feel better and be comfortable. Ask your palliative medical team for the pain meds to abate your active pains and discomforts…. It’s their job to keep you comfortable. You can drop those later when the ADT shrinks your Mets. Remember your bones aren’t what they used to be and most likely your oncologist will prescribe a bone helping drug infusion like Zometa to strengthen them as well. Ask him about it. Just IMHO, your palliative medical team should always be the authoritative last word.

I had lots of mets in my spine an very strong sciatica nerve pain.

My oncologist professor Epstein prescribed Firmagon injections and my pain gone in 7 or 8 days.

If you have mets in your spine the only ADT what you should use is Firmagon (Degarelix) otherwise you could have spinal cord compression.

I wouldn't use radiation untill your PSA is falling.

I started to think about radiation only now 4 years after starting Degarelix as my PSA started to rise.

what’s the downside to starting radiation like SBRT now instead of waiting?

I don't really know. For me it worked well as they installed the latest high precision SBRT MRI Linac in my local genesis Care 2.5 years ago.

Therefore I received a most state of the art treatment with the Swedish Elekta Unity MRI Linac machine which is located only 400 m from the place where I live.

My MO and RO agreed 4 years ago that I should see an RO ones my PSA starts to rise.

The RO ordered the PSMA PET CT scan and we decided to do the SBRT of my prostate only as at PSA 1.25 there was only cancer in my prostate with SUV max value of 14 and no visible mets.

I am still on Degarelix ADT injections only.

Your situation could be different.

The elekta Unity MRI Linac is a high precision SBRT MRI guided Linac which is up to a mm precise.

8 weeks after starting Degarelix start chemotherapy 6 cycles of Docetaxel.

my husband had a couple of tiny spots detected in 2013. He never had any pain. They are no longer visible. He has been on Lupron for nearly 10 years now with holidays every 12-15 months that last 6-7 months. No radiation, chemo or surgery. PSA undetectable. (It was 147 when he was diagnosed). Live your life, live your life. Good luck.
