Hello all, just wanted to give an update on my experience with Lutetium.

Although I have not had any chemo I decided to take a chance and traveled to India for Lutetium treatments. I started my first treatment last August with a PSA of 3.11 and a good amount of expression found by my PSMA scans. I traveled a total of three times with about 5 to 6 weeks in between each treatment. By the end of the third treatment my PSA had dropped to about a .66 and the expression in nearly all of my tumors was completely wiped out. I was very pleased. About a month or so after my PSA reached a level of .49. Now here we are about three months after the last treatment and my PSA has rebounded and I am now back up to a 3.08.

My doctor has recommended submitting bloodwork to Foundation for genetic testing. If nothing is found there he is recommending I try Xandi.

Any feedback or recommendations on my course of action is surely appreciated.

Where is the PSA coming from? Don’t you think you should have another PSMA PET-CT scan?

Agree with 6357axbx ^^^ that you get thee to a PSMA PYLARIFY PET ASAP.

I assume you were treated by dr Sen. What does she think ? I m in similar situation but on bicalutemide which brought psa down from 3 to 1 Have been considering India but have been unable to travel due to ortho problems. Hip replacement completed last week should solve that. Will be changing to xtandi shortly in hopes of less debilitating side effects while waiting for LU 177 here or there in India In the old days of radiation an initial drop in psa was sometimes followed by a temporary “flare” and then resumed the downward course. Wonder if that could be what is happening here. Best of luck

Wow, I have great hope for that if the Xtandi fails me. Doesn’t sound good

Reviewing your posts, you appear to have lymph node only metastasis and only one remains visible on latest PSMA scan after 3 Lu cycles. Though it cannot be expected to be curative, you could get an RO consult about SBRT to that one node to decrease tumor burden. (And did you ever have SRT to your prostate and pelvic nodes? Not clear to me.) Then perhaps another systemic therapy could control things. If not more Lutetium, then consider a chemotherapy or other AAR hormonal. Darolutamide is liking very good after the Zytiga fails.

I had EBR to the prostate and pelvic bed followed up with Brachy therapy to the prostate. That was 5 years ago. Based on recent PSMA scans still much activity on the prostate. Thanks for your reply
