Most of the medicines for PCa, with the exception of Provenge and hormonal therapies, have a deleterious impact on the bone marrow. Many clinical trials have minimum platelet counts to enter the trial. You may want to discuss one of the several BiTE or CAR-T trials with your oncologist.
Unfortunately one of the ways the cancer becomes terminal is by invading the bone marrow. Diffuse invasion of the bone marrow with depression of the bone marrow (low platelets, anemia, low WBC) is very difficult or impossible to treat.
Perhaps immunological treatments could be possible. One of the problems with CAR T cells is that requires chemo to deplete WBCs before injecting the CAR T Cells. You need to consult with experts working with immunotherapy treatments such as Dr Dorff :

I’m sorry to hear about your low platelet count - I have a thing called ITP that has given me low platelet counts for the last 40 years… usually around 30 - 50, but sometimes down to 9 - 12. I’ve gotten used to being bruised, and it has never been a drama. I was brought up to 100 by a week of pred before RRP, and when that only got me to 60 before nerve surgery last year, IViG infusion got me to 120, and I stayed there a couple of months.
Anyway, I’m now 65 and when it started to hang around 15 and my eyes went bloodshot, my haematologist put me on a drug called Revolade - I believe it’s sold under a different name in the US - and the impact has been amazing. Sitting around 100 - 130 ever since I started, with zero side effects.
It’s about A$ 3600 pm but is subsidised down to $40 in Australia - I’ve no idea what access to it is like in the US, but it may be worth you looking into this. The patent is coming off in a few months and Indian and Chinese drug companies are lining up to produce it.
