Looking for some advice. A close friend of mine now 81. Diagnosed 5 years ago. I am new to this as my diagnosis was 18 months ago. He had an RP. Path had nonfocal extra prostatic extension but margins clear. Seminal vesicle involvement. Node negative but only one node per side. I do not have original PSA level. No treatment was recommended. Now 5 years out and PSA bumped to .12 then 6 w later .16. Had a PSMA scan which as expected was negative at that level. He has a followup appointment with his urologist and I am trying to send him to my RO. Given high risk issues originally I think pelvic radiation in order. Still needs bone scan and MRI. If all scans negative for mets, would avoiding ADT be acceptable with a level that low? Is it required to continue to wait til his numbers get higher before starting therapy? I was surprised nothing was recommended post surgery but he did have negative margins, and he has done well for 5 years and is now a very healthy active 81 yo. Thanks all for any advice
