Hi all. My anxiety is back through the roof again with regards to my dad’s PCa - hoping the forum can help. My dad was originally put on zolodex 2 years ago and had 4 x chemo and radiation to the prostate. A new Met spot was found on his spine last year and so enzalutamide was added to his treatment. This Met spot has not disappeared despite the added enzalutamide and is now causing sciatica, so my dad is having spot radiation for it this week. My concern is why hasn’t the enzalutamide got rid of it? The met has actually grown since starting the enzalutamide - PSA now at 23. Does this show enzalutamide is not working? Or do mets sometimes need spot radiation as well? Thanks all. An anxious daughter.

I have the same issue. After 2 years of ADT and spot radiotherapy, my T9 tumor has grown by 50 percent. I have 2 consultations this week. I will be watching this thread.
i would request a direct biopsy if possible, to do genetic, IHC and histological studies to determine if the cancer may be treated with other drugs, such a keytruda , olaparib or similar or chemo with cabazitaxel and platinum compounds. If biopsy is not possible he could qualify for treatment with Xofigo (Ra 223) which is effective for bone mets. If he is on denosumab or zoledronic acid , he could continue taking enzalutamide during the treatment with Xofigo.
It shows that that met is enza-resistant.
