Need info from the brains for an additional drug Kaiser wants me to add to the heap.

Kaiser just contacted me and wants me to come in for a course of treatment with Evushield ( which is Tixagevimab and Cilgavimab ) , which is being prescribed for me by “ emergency use authorization “ because it’s not approved for my application in the USA …

I don’t think I’ve ever heard of this drug / treatment on the group before , might have missed it. Plus this kind of attention seems somewhat out of the ordinary, from Kaiser. Anyone know about this drug being used , investigational , related to PCa or maybe this is just trying to bolster my immune system against bugs like the flu or Covid. I know nothing about monoclonal antibodies , or why Kaiser might doing this ….. any thoughts, contradictions or ???

My husband received it! He is currently receiving chemo, carbazitaxel and carboplatin. Because this diminishes his immune response, Evushield was recommended. One of his good friends who has had a kidney/pancreas transplant also received it. No side effects, except for peace of mind. Knowing his WBC and neutrophils drop down low and he has little natural immune response, this has enabled us to have friends over, travel a bit, go out to dinner, etc.

Whew, I saw your post and hurried to read it. Glad it isn't about progression. This is interesting, mostly that they contacted you for it. Let us know what you decide. Not sure what I would do in your case so I can't offer any insight.

What are you on besides Lupron and Zometa? If thats it and your WBC counts are normal I would wonder as well.

I’m only on Lupron Xtandi and Zometa and in the last couple monthly tests, my wbc counts have eked back up over the bottom chart line into actual low normal. I had a continuous 7 month “ dive “ of most all my tests ….but recently ( last month ) that seemed to break up somewhat and some stayed kinda constant at just below low normal and others reversed direction and crept up a bit rather than continuing the decline. My kidneys not very happy with my microalbumin climbing , about 200 or so right now. Dunno if that relates. I’ll schedule a video visit with my GP and ask her to explain why I need this. Hopefully I can get some talking points to ask about and some insight.

It’s pretty unusual for Kaiser to reach out to me like that, especially knowing it’s currently an unapproved “ emergency use only “ medicine. And one can wonder about Kaiser wanting to administer a very expensive drug , the whole course of injections costing several thousand dollars. That with a tenuous justification. Its puzzling and if I were a suspicious person, your explanation puzzle piece does seem like a plausible fit. Lol

Possibly a lot has to do with a person’s initial diagnosis. Once that diagnosis , from a team of doctors , gets on your record… in Kaiser’s eyes you are always “ that “ state until the record changes. My diagnosis is basically “ end stage , stage 4 PCa, inpatient hospice “ …. It seems like a lot ( all ) of what Kaiser does hinges on that initial DX. ( duhhhh huh yayahahahaya ). Maybe Kaiser is bouncing off that DX and thinking that in my diminished state that I’m especially susceptible to harm from Covid and are acting out of an abundance of caution. When Covid vaccine first became available, they did a similar “ very energetic “ effort to get me in for my 3 vaccinations.

Heck, I don’t even have an oncologist, right now, and haven’t had one for , maybe, a year or more. My oncologist just up and disappeared without fanfare or notification of his patients. I discovered it accidentally, second hand, when they were trying to get someone to sign for my Zometa infusion in the infusion clinic. They are trying to get me another oncologist soon as they are able to hire one. They have three now, but they are overloaded. Maybe , just maybe …Kaiser wants to make double sure I don’t catch Covid and croak … and how bad it would look in that respect. Covering themselves, so to speak.

I’m waiting for a video visit with my GP …. Hopefully that will clarify everything…. Knowing Kaiser tho … it’s likely to only make things murkier yayahahahaya. Oftentimes it seems that I get more information and clarity from members of this group than I do from Kaiser.

Evidently this is only a prophylactic move by Kaiser, I was hoping that those antibodies might have a beneficial angle for my PCa … seemingly not ..judging from group response. Still , thanks to the guys here, there doesn’t seem to be any downside and maybe I’ll have “ super “ Covid protection … Might as well go for it , if it’s not going to make me feel sicker than I already am. It will be interesting to see what's up. I think between the group and yourself you have covered and narrowed down the possible reasons.It probably started with a corporate memo to assure as you said if its available dont want to lose someone and be left with any responsibility. Yea this prophylactic will beat a hazmat suit.

I think the first reported covid case(s) were reported in Washington State.

Then next 2 people in Chicago suburb day before my first chemo infusion in Chicago suburb. I was glad that my insurance didnt cover the Oncology dept. in the hospital I went to. The specialty cancer (cancer only) place I went to I felt safe thru chemo. They were super cautious and of course unlike a hospital did not receive covid patients so I felt relieved. Part of the journey. Part of the weird, interesting journey. The evolution of covid spread during my 5 months of chemo. Unforgettable.

We had extreme Covid infections here in Central California …. The whole 1.2 million metro thick with Covid deaths …. all hospitals jammed beyond capacity, rows of refrigerated trucks full of bodies. it’s still pretty hot with Covid cases. Central California is strongly red state ( unlike the most populous parts which are blue ). , masks are unthinkable for most , only 42% of population here immunized. The rest refuse to get immunized or wear masks unless forced. It’s a kinda scary place sometimes yayahahahaya.

It’s like everything is bake to business . Concerts , Coachella , no masks many got covid . Some would rather fight than mask up . I don’t get it . But im unvaccinated . I trust not pharma . IV social distanced since dx pre covid . If Kaiser can’t find docs ? is it going under?

Kaiser is frugal ( can you say cheap ) , they have trouble finding doctors that will work for what they want to pay yayahahahaya.

I had Kaiser as a kid growing up in nor cal . They fixed my broken elbow pretty good! My buds son is a top dog executive there . He makes bucks ! He a great kid maybe 45 now . Good luck with Kaiser .

I was the guy work mates came to when they couldn't spell.

About a year ago when spell check could not figure out what I was trying to spell after several attempts on my part I just stared out the window and said to myself wow this is crazy.

I find it interesting. Who knows maybe finding stuff interesting is a defense mechanism to keep from getting bummed.

Actually I went for being a Navy Corpsman as the medical field I found very interesting. Now I am the center of interest lol.

Poor dogs. Our last dog was a female.One of her medications came from Walgreens. Was a trip to see her name with our last name on the label. Really further drove in the feeling of "she's family."

In Fresno … I was the Director of Biomedical Engineering here in town at the V.A. Medical Center for 22 years and same at Darnall Army Hospital , Ft Hood, Tx .. nearly a decade before that.

I used to go to Kaiser , after I retired, and see people that I knew or who worked for me …for years after I retired. Yayahahahaya.

The drug is for immune compromised people, it's used as a protection against Covid etc. we are desperately hoping that it will be approved in the UK too, as our youngest daughter has no immune system whatsoever. Covid and all its associated viruses are an extreme danger to her.

Your immune system will be very low, so this drug could be a huge help to you.

Phil I am sorry to hear that. Daughter with no immune system whatsoever. Some voices are raising but to no avail here in the states by the immunocompromised and families of."You forgot about us". No masks. No distancing. Come back to work in the office.

The parent can bring it home to the immunocompromised child or partner, family member. It makes life difficult I am sure.

Thanks very much for your reply, yes it's absolutely dreadful, she doesn't dare go out much anymore, school is an absolute nightmare, as its been rife with it, her only son has diabetes type 1 and she is on her own with him, the father cleared off when he was diagnosed at about 16 months old, he's now 10. Our grandson has been the only class member to wear a mask, but due to peer pressure has asked his Mum if he can stop wearing it now.

You are so correct, they've been abandoned, people don't consider them and neither does the government, it's as if Covid was just imaginary, now all the security has been lifted.

We are hoping to see them this year, the first time in three years, but Evusheld would have made life so much better for them.

Yikes , that’s got to be gut grinding with your daughter … my heart goes out to both of you. My beautiful daughter passed in her 30s( suds … sudden unexpected death syndrome ), very healthy , just keeled over in front of her computer one Christmas Eve , a few years back…. I understand how you feel. The evushield is just two injections , maybe you could fly to the USA or a nearby country and get her the shots. They are very expensive I believe from what I’ve read on the web, but financial help is often available if you dig into that help and some guys here on the group know a lot about that. Evushield isn’t approved in the USA yet, only in emergency cases ( I’m an emergency case ??? Wtf yayahahahaya )

My immune system is working pretty good right now, I believe those shots will be prophylactic in my case.

Kaliber, I've just read your response to my wife, I struggled to finish it with out a blub, must be the hormones.

We are so sorry for you and your wife, nobody knows what goes on in people's lives, until sometimes like this. It often appears so cruel, instead of the happiness it should be.

We do worry very much about her, she wasn't diagnosed with it until we had left the country to live in France, she contracted a virus and afterwards she had no immune system, she has IV every 4 weeks. It's been a nightmare for her as she has had to bring up her son on her own, he's type 1 diabetic, during all of this and being so far away I got hit with this.

We are seeing them for the first time in 3 years, this coming August, we are worried about the flight, that's where Evusheld would have helped her and in so many other situations.

You're a great guy, we send our love to you.

Hi Scott, thank you for your kind thoughts. So do we, but the government is debating it, which means it may not happen at all. They have been very good at cancelling Covid and ignoring the health of the needy.

It's our greatest wish, that she should be able to live her life normally.

My wife received two doses of the Evosheld. The first was just as it was released to physicians. The second was many weeks later when "they" decided the first doses they were giving were too low to be effective for immunocompromised patients. No noticeable side effects.

The dose was increased when Omicron became prevalent. It is so much more transmissible that a higher dose was needed.

I had Evusheld a month ago while on chemo. No side effects. My wife got Covid last week (on the day I received chemo) but so far I’ve been ok - so maybe it works for me.

Roger that … My local Kaiser is offering the 4th shot now as well,… I have the first 3 . I guess this treatment is better than the regular Covid shots , supposedly it lasts a lot longer and makes a lot more antibodies. I’ll talk to my GP about it but sounds like it might be advantageous to go get that evoshield. Thanks ❤️❤️❤️

My husband has prostate cancer and CLL. Due to the CLL, he has no immune system. He received Evusheld on March 31. Evusheld is a MAB that is administered in two injections, supposed to be in the buttocks. The current belief is that it's effective anywhere between 3 and 6 months, but no one is certain yet. My husband had zero side effects to the injections, but then he had zero side effects to the first two Pfizer and subsequent two boosters, either. It's for immuno-compromised. The US government has not yet funded Evusheld past the end of this year.

My recommendation is a mirror image of whatever you choose...

Lets hope that the Hippocratic Oath still holds merit: "A solemn promise: Of solidarity with teachers and other physicians. Of beneficence (to do good or avoid evil) and non-maleficence (from the Latin 'primum non nocere', or 'do no harm') towards patients."

"Kaiser" isn't a doctor! I strongly urge you to establish with doctor(s) you feel comfortable with. Then, you'll have someone you can trust, who knows you and your entire medical history, to ask these questions. I've been a member of Kaiser since I was 4, which means for 73 years, so I'm quite familiar. Yes, it is a big bureaucracy, but that just means you need to advocate for yourself. Insist on having a Personal Care Physician (PCP) as well as whatever specialist(s) are applicable to your needs. They will provide them if you just ask. And if you don't like the first one, they will give you no guff about changing . Remember that the squeaky wheel gets the grease. You need somebody who is your trusted partner, and advocate, thru all this.

At my Kaiser , I have a team of 5 to 7 doctors + several hospice personnel overseeing my treatment, with my oncologist and GP being the ones that communicate with me the most. GP most often. They were assigned, not sure how your Kaiser handles this. I have an elaborate array of medical personnel managing things like my ADT induced high bp and my ADT induced high bg as well.

As for being providers .. I just discovered my oncologist left unceremoniously about 8 months ago and told none of his patients. Kaiser never told me, I found out by accident. Currently this Kaiser has 3 overloaded oncologist and is trying to hire one more . I’d be in his / hers patient list.

I’m on here asking about this 1, because it sounds odd to think I’m immune compromised and 2) Was hoping they found some connection between monoclonal antibodies and PCa and wanted me to have this just in case . I was hoping someone here knew of an obscure connection between PCa and the antibodies and possibly … I could get some talking points.

Ultimately I’ll now be asking about why they want me to have this ( evidently ) Covid treatment instead of my 4th shot. I’m trying to establish a video meeting with my GP but our doctors are seriously overloaded and she has no openings in the next 5 -6 weeks. ( or more , calendar only goes out that far ).

I always go thru my medical team for any final decisions and choices.

The request to have me consider receiving the Evushield was sent to me signed by my GP.

Sorry if I was confused. You had written earlier that you didn't have an oncologist and hadn't seen one in over a year. That's what prompted my response. I'm very happy with my care at Kaiser, but do find that sometimes I need to be a strong advocate for myself.

You are absolutely right …. My personal oncologist flew the coop …. Might have been about his medical degree from Tiberia. Dunno …( yayahahahaya). I’m fairly low maintenance right now but I still require an oncologist to order my blood tests , order my meds, and give my results a quick glance …. The other oncologist do that or assign someone to do it. I hate to think what happens if I suddenly go sideways tho ….

For me, it has been really important to feel emotionally supported by my oncologist. There are so many ups and downs in this game. She involves me in every change in my care, and is very open (even when she disagrees with the conclusions!) to the articles and ideas I grab from this Web site and throw at her. She's in another city, but these days all our conversations are online anyway, so that doesn't matter.

Hey Kaliber: beautiful pic of your truck up in snow country-- love the high Sierras-- many fond memories of skiing up at Mammoth and Tahoe. Anyway, you probably know WBC and neutrophil levels are a good indication of how immunocompromised you may be, and a guideline for immunity against CoV is the antibody test given by LabCorp for about $10. The high sierras are just a short drive from here … over the decades we’ve spent countless time up there …. same with the coast. It used to be so beautiful up there before it changed. First the forests turned brown all over when the bark beatles and continuing severe drought killed nearly one out of every three trees… in places ( groves ) even more …. Then came the vast fires that swept thru all over the place. Nowadays you have to become accustomed to a newly changed and different Sierras. We all love to see the thick green fragrant lush bushy forests, but the disheveled way it looks now is just a natural phase. Quite a bit of the bushy look still exists in places.

I’ve had three of the Covid jabs and I’m waiting for a return call about getting the monoclonal antibodies….. I’ll know more about why when I talk to them. WBC’s ok.

I guess I contracted Covid about two - three days ago, the hammer brought it home. She got sick first and then about two - three days later , now I have it. I’m not projectile vomiting and as sick as she is ( she has heavy smoking and drinking co-morbidities )…. at least. Just minor symptoms compared to my PCa load , hopefully those three immunizations will temper the hit and I’ll slide right thru this with just minor difficulty. Covid is rampant here in red state central California … only 43% of population immunized and many people refuse to wear masks . Nightly news tonight says that sewage monitoring of Covid has detected a massive jump of Covid this week.

Interestingly…. Covid has frozen my blood sugar in the 170-180 range and it doesn’t go up and down it’s normal daily excursions ( I check my bg 1-3 times daily ) and my apnea has skyrocketed. I’m usually a ahi of 1.4 to 2.5 but last two nights my ahi has suddenly jumped to as much as 17. Of course I’ve got the regular symptoms of diarrhea, headaches, super nausea , heavy body pains, a little breathing resistance , no fever but feel chills a lot. My taste buds trashed and food textures can make you gag and spit out some food while eating. My appetite is shot, only a few mouthfuls of food in three days.

I’m using a special sativa to kill the nausea and kill the body pains , its like a magic bullet and makes nearly all the symptoms vanish until it wears off. I’m crossing my fingers that it doesn’t get much worse. One of my best friends , Victor, passed from Covid this past New Years ( Christian minister, wouldn’t wear a mask and dismissed the immunizations) … I’m definitely a little apprehensive, especially about the hammer making it thru ok. It’s always something, isn’t it ?

Speaking of Vegas, right before the hammer retired last year , she was in Vegas for a work medical conference …. I looked at her phone in the “ find my phone “ ( ios ) app and it said she was in the Las Vegas police station parking lot for 5 hours. I asked her about it when she got back , she said the app was probably malfunctioning ….lol.

I have 3 as well and considering the offered 4th. Our main concern is our son bringing it home from school but so far he has been doing well with masking and made it through the last spike so hopefully it will be the same for this one. My wife had a bad response to her first 2 jabs and refuses to get anymore.

When she’s ( instantly ) done talking about it, best to move on and let it drop. 😂 I probably can scrape around in some dark corners and find a bit more of that Jimbo aaa sativa ( wink wink , nudge nudge ). Originally, years back .. that strain came from one of my prized mothers but Jimbo hybridized it down to a dazzling 5th gen product … the flowers resemble fingers , like a buddhas hand citrus fruit. They are thick , heavy and very resinous. We always accused him of hanky panky with his plants and a nearby orange grove yayahahahaya. It’s been giggle weed from the beginning, but its profound nausea crushing powers and impressive light to moderate pain control are only recently appreciated.

Man … I can’t imagine kids in the house during this Covid pandemic … my imagination would see them swimming in a soup of Covid bugs at school … every time they came home would be scary … it’s hard enough with two people isolating themselves, and …obviously… even that will fail. Just think of all the additional exposure. You guys are brave .

Well, if your wife doesn’t work with the regular Covid jab, maybe these monoclonal antibodies would work for her. Everything I’ve seen says there are no side effects, except that third small arm that grows out of your back. ( no, just kidding ) .

If your wife is still vomiting, have her doctor prescribe dissolvable Zofran. Make sure it is the dissolvable form. Something else that's really helpful is Liquid IV. It's an electrolyte solution that you add to water. She's probably dehydrated by now. You can get it at Walgreens, Costco, etc. Those two things really helped my son-in-law when he had covid and couldn't keep anything down.

Roger that big guy … I’ll save this info, just in case… you never know when you’ll need new or different approaches. Right now she’s using a OTC ( buy similar in local corner stores ) sativa cannabis flour . This product is “ extremely “ effective for nausea plus it adds on a strong mood elevation , appetite enhancing and powerful pain relief. All in one peanut butter treat ( mix the flour in PB and eat it ). It’s a nausea sledge hammer. She’s also drinking Pedialyte as well. Three hours a day in a jacuzzi can drain the water out of you ( sweating ) … we always have those sports drinks and Pedialyte on hand, works out handy in this instance.

Yea as they say everyone is done with covid but its not done with us. Makes it hard to keep it away from us who don't actually (in my case and I assume yours) want anything to do with it.

At first you described it as light but as you later went into your actual symptoms you got hit actually with quite a few symptoms.

I might have missed it in the thread but do the hammer or you have a fever? I would think so and that would account for some of the symptoms but yea you got some of the classic ones.

The hammer has has an elevated temperature ( 99.8 ) not serious yet …. Myself I dropped a bit from my usual kinda low ( 95.4 ) temp and I get chills. My PCa symptoms are so over the top, it’s hard for other stuff to register much … my bg moved , finally ( dropped 25 points ) ..today, first time in days it hasn’t been frozen at slightly over 180. My apnea has dropped back closer to normal too … but is still elevated. Got hefty nausea, light headaches, taste has changed still. That scary little breathing issue( gasping for air, or sense of not getting enough air ) is quickly dispelled with a handy , inexpensive, finger oximeter and chlorpheniramine . Xanax cuts the breathing panic tendency, that it provokes, completely.

My total loss of appetite remains, but in my case prolly a benefit. My Watch is loose and floppy on my arm today … before the Covid, it was a bit tight. I like that. There’s a lot of Covid exhaustion that stacks on top of the ADT tiredness and exhaustion… no biggie tho … manageable… so that’s good. All in all, I’m thinking I’m “ real “ lucky I have the 3 jabs .

This seems “ do-able “ if it doesn’t ramp up in the coming days … I’m about 3-5 days behind the hammer. I watch her close and keep my fingers crossed. She has hefty drinking and smoking co-morbidities ( lungs shot ) …. But she’s a trooper and tough as shoe leather … losing her would be utterly catastrophic at this time of my life.

Cripes this is the “ chits “ isn’t it brother camp ? There is no way in hades that us PCa guys ( especially the ones suffering daily ) want or need this Covid crap dumped on top of the PCa , yet there it is and here I am.

Maybe speaking of my Covid adventure and it’s parts will be useful here on the group. Prolly an interesting topic for a lot of us.

That's the line I needed when my son asked me if I had any SE's from jabs.

I started to explain I'm pretty zoned out with PC and treatment and cant really be sure what's, what and we both laughed. He got it.

Hope fully stays in that category and not elsewhere but regardless who needs the what if stress.

When I say my PCa symptoms , it’d be more accurate to say my SEs from ADT are over the top “ yayahahahaya. I just lump it all together. I hear you about mental instability and clarity … I’ll bet someone could write a book about the difference angles in PCa. It’s a big topic. You see it from dozens of perspectives here on the group. It’s rough on families and us guys ….. either the stress or mostly the mental SEs from powerful life altering drugs. They can make your speech slur and fog some thinking … never mind short term memory loss ….never mind short term memory loss … I try to ignore as much as possible, but its all part of the rich texture of life.

I came to Calif in the winter of '98 when we got 57" of rain in the Bay Area in the last great El Nino and everywhere was flooded, and now living in San Diego where we get single digits and it's dry as a bone. I remember a NorCal local telling me we never get an "average" year of rain-- it's always too much or too little. Now the extremes are worse. Noticed the Salton Sea is drying up with coyotes scavenging anything left. It will soon be like the Salt Flats in Utah.

Well damn, sorry to hear about getting Covid. Two of my co-workers at Hq up in the Bay area just got it last week, and they are saying it is now one of the hot spots because relatively fewer people have been infected. Although highly vaccinated, the vaccines' effectiveness are waning.

Hopefully, you and the hammer got the less severe Omicron variant, and this will pass soon. That's a huge increase in your ahi score, I guess your body is working overtime to fight it. I think if I got it I would take the Paxlovid pill as a precaution and try to stay out of the hospital. With still low WBC's from radiation, I would be high risk.

Well, at least you'll have super-immunity when this is over-- three vaxes and a light infection is where you want to be. I tested my antibodies a couple of months ago and they were off the scale from three full Moderna doses, so I'm holding off on the fourth vax until there's something new out, like the oral vax or nasal vax.

Keep on chuggin' buddy, when you feel better, get out, and take that nice truck for a spin! Hope this passes soon for you and the Mrs.

Ryte on …. Here in the Central Valley we get around 11” of rain on a good year .. less than half that so far this year and the rainy season is about over. Our reservoirs are really low too due to serial drought years and lack of run off. Farmers kinda freaking. I was worried that the Covid would intensify as it matures and I’d take a dive but that hasn’t happened so far. It’s been fairly mild for me but the hammer has had it worse, somewhat. She’s ok tho, so far. It’s not like it’s enough just with the PCa and I’ve had two “ really “ close PCa calls recently …. Wooohooo let’s pile on a killer virus just for ducks yayahahahaya. You just gotta love it.

I do have the offer of the monoclonal antibodies… like you said, my three Covid jabs, the extra antibodies from exposure plus the monoclonal antibodies …all that should combine for some super Covid protection. Can prolly still get Covid again .. like right now …but should be hardly noticeable ! I’d guess / hope.

I’m not a pickup truck kinda guy , I’m more of a suburban with twin back doors type….. the story of how Chevy gave me , outright , that truck … a pocket full of “ oh $hit , we’re sorry “ money and a $1200 kayak trailer ( lemon law ) is a long one ….

Yes, I drove down the I-5 last week and it was brown grass as far as the eye could see. Saw some signage posted by the farmers about water rights, not enough being diverted to the farms. Not looking forward to this fire season, I think we're in trouble.

I love vintage autos, driving two 20-year olds now-- Toyota and BMW with a combined 300K miles on them-- best way to beat the costs of driving in a high-tax state if maintained properly.
