Low PSA High Testosterone

I'm wondering why is my psa level low( 0.1) and my testosterone level high (699) with the drug Xtandi?

If you do not add Lupron to Xtandi, this is what you can expect. Xtandi blocks the androgen receptors of the tumor cells so that the testosterone cannot trigger the tumor growth. This way testosterone rises above its normal level. Xtandi has additional effects against the tumor.

I would not add Lupron to Xtandi as well, to improve quality of life. But Xtandi is approved in combination with e.g. Lupron only. So almost all doctors will apply it this way.

BECAUSE ..Extandi is an Anti Androgen medication...meaning it ONLY blocks the effect of testosterone . It does NOT suppress testosterone. Therefore , PSA goes down But testosterone remains high. Bicalutamide also does the same thing.. reduces PSA but keeps Testosterone normal.

BTW, Congratulations on PSA 0.1...This Nadir PSA foretells many years of survival.

With a PSA of 0.1, there is no need to add Lupron type meds at this time.

One of the benefits of using an androgen receptor [AR] antagonist (any of the "lutamides", such as Enzalutamide [Xtandi]) as monotherapy is that one can enjoy the benefits of normal testosterone [T]. You are fortunate to have normal T that is double or greater than that of many men at diagnosis.

Are you only taking Xtandi? That is unusual - Usually one takes both a drug to stop production of testosterone (like Lupron) along with Xtandi.
