Should I try Zometa for Castrate sensitive Bone mets on ADT

Hi gents.

My specs: 52 years, Stage 4, Lots of bone Mets and pain. Dx April 2020 on ADT Orchiectomy and Zytiga. PSA came down from 300 in April, now 0.1. However bone pain persists.

Visited a new Dr, and he recommends Zoledronic acid infusions.

I am Castrate sensitive according to the definition, but scans show some progression despite this fact, in my bones. Sternum and ribs.

My question is: Should I start on Zoledronic acid now? There are so many side effects, and conflicting theories on when to start or not to start. Your advice is appreciated.

Zometa will strengthen your bones and prevent the fractures caused by weak bones. I suggest you get a DEXA scan first to see if your bone mineral density is low. No point in fixing a problem you don't have.

Thanks for the reply.Getting a DEXA is exactly what I also thought would be the first step.

I would discuss a Xofigo therapy with a nuclear radiologist. This therapy can increase your overall survival as studies have shown. Zometa will just prevent fractures.

Well, we're all biologically unique, but I've had no side affects from Zometa. Prostate Cancer cells loves to anchor on bones, this is their favorite home...

I would get on the Zometa in that situation even if the Dexa scan is not abnormal. Your bones are not normal and Zometa can reduce “skeletal related events “, pathological fractures end even further bone mets. I would combine this with Celebrex, a favorable combination. And if side effects to Zometa are too bothersome switch to Prolia. ( for further discussion with your doctor).
