Zytiga vs Docetaxel Sitrep and new question.

Hola Shipmates, I have encountered some choppy waters and need further advice.

To review: Diagnosed in June 2018 with PSA 178ng/dl and Gleason 9 a

t biopsy. Started with Casodex followed after month with LUPRON. Reached PSA nadir of 0.9 in summer of 2019. PSA doubling time approx ~ 2.0/2.5 months. PSA July 20, 2020 = 116ng/dl. T = 6.0ng/dl MRI showed mets to spine and suspicious ganglion activity.

I have now have additional info from office visit (July 5) And OLAP surgery ( July 8) both with my Urologist.

In July 5 office visit Urologist favoured escalating to Zytiga. I told him it was Prohibitely expensesive for me, so he suggested 200 mg Ketoclonazol (3x a day) Plus 5mg prednisone (1x a day). Although Ketoclonazol appears to be less effective than Zytiga and may have some more side effects, I opted for the Ketoclonazol which is considerably cheaper than Zytiga. ( 5 week supply for 380 pesos which is less than20 bucks.

Post surgery, the Urologo advised that the tumour had engulfed my Urethra and that was not a good sign and that given a reduced life expectancy Zytiga was now the clear choice because of quality of life factors.

So i investigated purchasing Zytiga from India where it is consideravbly cheaper. Previously, I have read favorable reviews about “All Day Chemists” but they do not handle Zytiga. Do any members of this group have recommendations about any other suppliers in India?

Is docetaxel expensive for you? Chemo with docetaxel plus ADT has similar results to ADT plus Zytiga. You should try to get an oncologist who specializes in prostate cancer and let the oncologist guide your therapy.

Yes, I intend to have a consultation with an onco, but from what Others irell me, chemo is very expensive here and it has to be prepared by a specialist and administered byan expert.

Hello,

I agree with Tango65. You need to ditch your urologist and find a competent medical oncologist. Nizoral (ketoconazole) is an anti-fungal medication used to treat a number of serious fungal infections. I am not sure of its application for prostate cancer. When you say chemotherapy is very expensive here, where do you live? I think your best options are Zytiga (abiraterone) plus Prednisone, Xtandi (enzalutamide) or chemotherapy (docetaxel). A good medical oncologist might also be able to help you with funding sources. Good luck! Hope that helps.

Both ketoconazole and abiraterone inhibit CYP17 which is what the adrenals use to metabolize cholesterol into steroids and hormones (most importantly for prostate cancer, testosterone). It appears as a standard treatment in a prostate cancer book I have that was published in 2010. You can google ketoconazole and find lots of references for its use in prostate cancer. Your urologist probably suggested this approach because of its low cost.

I’m in USA. There are foundations that can help enormously c costs. My health center’s “speciality pharmacists” just took over my payment, and I haven’t had to worry about 2nd generation ADT costs. They also got enormous and very surprising help from my medicare drug supplemental provider. I leave day-to-day drugs to my chain pharmacy.

David, not sure if you had any responses for your enquiry about sourcing Zytiga from India. I'm still trying to do exactly the same thing. Two possibilities you could try to check out are:1. Fortis Memorial Research Institute ;

2. A pharmaceutical company link address : magicinepharma.com. Get prompt WhatsApp response from them.

Ronnie, I have received three shipments from India and am very pleased with the service I get from them. The first shipment was a three month supply sent to USA by FedEx, the next was a three month supply sent to USA by India post, the third was a six month supply sent by India Post (EMS). EMS is faster and cheaper than FedEx. I am very pleased.with them and their product.

Regarding the product. For my first month I used the real deal. This gave me a baseline of what I could expect from Zytiga. When I switched to Arbiterone Acetate generic from India my PSA continued to drop.

Did you have any problems in getting the drug through Customs at your end? The impression is EMS is the faster and cheaper shipping option.

Could I ask what price they charged you (including shipping) for the Abi and how was payment handled (e.g credit card or bank transfer).

I have recently been in Whatsapp contact already with some of the companies in India but its really impossible to know if I'm dealing with a genuine organisation or a fake website. Discount has also been offered for multiple unit purchases!!

Really great to hear that the product you purchased has been getting your PSA to drop.

I have just started taking the 500mg dosage with low fat breakfast and hope to see good results at the next meeting with my doctor. I'm hoping to eventually reduce the dosage down to 250mg/day with low fat meal.

You can take 1 250mg dose of Zytiga with food opposed to the full 1000mg w/o food to save money. NCCN Guidelines added it as acceptable due to "cost toxicity" which sounds like where you're at. Just a thought.Also Janssen offers a program that pays up to $10,000.00 in co-pays if you are eligible. I was approved only to find out Kaiser supplies me with generic abiraterone so no need and it only applies to non-generic Zytiga, generic has not been a cost burden for me up to this point.
