Urethrotomy with Mitomicin C injection for Radiation Stricture

Hi, looking for information from members who have had Internal Urethrotomy done with the injection of Mitomicin C injection or member who may have some knowledge regarding this MMC on stricture.

Not a whole lot of study on Mitomicin C (which is normally used in bladder cancer?) on the internet. One of the urologists I saw proposed a repeat internal urethrotomy but this time with the injection of Mitomicn C to help reduce the regrowth of the scar tissue.

It has been used to treat strictures in esophagus and larynx. It appears to be safe and effective.

Thanks Tango, not sure if we can compare the two applications, my urologist never mentioned side effects and complications associated with its use on urethral stricture which is my main concern and am seeking information or first hand knowledge of others.

I have a urethral stricture within the prostate following HIFU. I had a dilation, and I catheterise once a week to keep it open. I have just started taking serrapeptase 50mg 2x a day to see if it helps. Together with all the other potential benefits in reduction of inflammation and removing fibrin in general I think it's worth a go. Let us know how your treatment goes. Cheers

I had a dose of Mitomicn C for scare tissue that was continuously causing strictures after a salvage robotic PT. My oncologist was using a laser to trim the the scar tissue prior to the Mitomicn C. The injections really hurt me. I could not go to work for two months after the injections. The injections were performed at a major cancer center in Texas. After the injections I started getting a hard, crystalline buildup in my bladder neck that had to be removed every 8 or 9 months thereafter. I believe the area would have healed better without the Mitomicn C.

Wow, thanks for the first hand info. Now I think I am going to have a discussion with my urologist on this.

When was this done? Do you know the dose that was used?

I asked the urologist did the injections, she just said I took a small needle and injected all around the bladder neck. She gave me no dosage. The procedure was performed about 6 years ago.

I did not mention it but 4 years before the PT I had 39 proton radiation sessions which "cooked" the tissues on my urethra and baldder neck. I asked the urologist who performed the injections if she had ever used Mitomicn C after proton radiation and a salvage PT. She said no, I was her first.

How are you doing now and do you have still have stricture?

Do you feel that Mitomicin C helped you with the stricture issue?

Would you recommend it?

My stricture is related toHigh Dose Rate Brachytherapy and in addition I also had IMRT.

It is hard to say. I have not had a pure stricture problem but the bladder neck pain buildup and pain still had to be addressed every 6 to 8 months with the laser procedure.

However, my local urologist tried something new 14 months ago. He set me up for ultrasound ablation as you use for kidney stones. Since the ultrasound, I have been clear of all pain and crystalline buildup.

I have been on eligard since 2012 and erleada since 2018. My PSA has remained undetectable.

I am caught between a rock and a hard place, between temporary fix internal urethrotomy and possibly a long term fix of urethroplasty. One of the urologist proposed the rethrotomy with Mitomicin C injections but I could not find long term study or have not heard it being commonly used. So there is a lack information. The first study was in 2007 but since then small studies (perhaps not in the US) and always wonder why is not being commonly offered.

Complications and side effects were not really discussed in those studies. Scared the hell out of me.

Thanks for you help. I am leaning toward declining the MMC injection because of the unknown risk.
