Hi Nalakrats. Yes it’s a small study but seems very interesting. A friend of mine is undergoing privately paid for Lu177 treatments in Germany and the doctors also informed him of this. So they’re well aware of this.

There is a trial at St Vincents Hospital in Sydney where Xtandi is used throughout Lu177 shots and where men have had chemo and after Xtandi or Zytiga failed.

The chemo can make Pca sensitive again to these drugs and increase PsMa expression and increase SUV for Ga68 in scans, and then for Lu177 in therapy, ie, make more Lu177 gather around Pca cells for the same sized dose of Lu177. This supercharges effect of Lu177.

But its theoretical. It may have happened to me after Xtandi was added after 3rd shot of Lu177. Psa went from 25 before Lu177 to nadir of 0.32 12 months later, but is rising again, now 1.5, maybe Xtandi is failing, but CT scans show only 2 active bone mets.

Could they target those two bone mets with SBRT?

Possibly yes. But it is too early to decide way forward yet. Maybe more mets show up soon and systemic treatment with more Lu177+Ac225 could be used.

Meanwhile, I did 87km on bike last Sunday, and today, Tuesday, I did a good fast 60km and felt extremely well. I am getting stronger after having stomach blockage and 8kg weight loss in 11 days at hospital. Pca is not my big worry now. Nothing is a big worry.
