Been about 6 months since Xofigo was too tough for my body. On weekly taxotere now, 3 weeks on and one off. They are thinking I need to get things drained in chest. Have not been able to be as mobile yet from radiation for met hot spots in ribs and then Xofigo. Still on Lupron 6 months shot and monthly Zometa. PSA is going down since chemo (79) from 120’s Already had chemo in 2016 when dx with 642 PSA with bone Mets Gleason 7. Did the xstandi and zytiga. If chemo is causing effusion, what is next best options? Do not do well with radiation.

Are the pleural effusions from chemo or from the cancer? Discuss adding Celebrex to Zometa. Have you had Provenge?

Since you have been in most of the drugs and if it is affordable, you could consider Lu 177 PSMA or Ac 225 treatments in Germany or in Australia.

There are a few clinical trials in the USA with Lu 177 PSMA and one with Ac 225:

Ask about doing cytology in the pleural fluid to see if there are cancer cells it could help to determine if the effusion is caused by the chemo or by the cancer. If they find cancer cells I believe they will consider that the cancer is causing the pleural effusion.
