Update after oncology visit

MO offered us a trial of olaparib in combination with AZD6738 . Husband did not have braca or any other gene mutations show up on DNA testing so I asked why we would go on this. He said "if it works, it will help bone mets" . It is a phase two trial to check efficacy in patients without gene mutations the theory behind this is the new drug will activate the olaparib into working on genes without mutations so that is what they are testing for . Still mulling this over but feel that we are in a position where psa and alkaline phosphates are both rising (84/169) and scans show progression in spine, pelvis and ribs, I'm leaning toward not jumping into this trial but getting RA 223 started soon. A request for referral to nuclear medicine was put in by our MO and hopefully we will meet with them soon. Also again discussing getting tissue sample for any trials going forward including for a PSMA scan. Also discussed out of pocket PSMA scan to think about LU 177 in the future. I

Side effects or olaparib? What were they. Eileen reading it almost looks worse than chemo.

Anemia and weakness. Does not always bring psa down.

If the cancer does not have mutations in the dna repair genes, you should consider the possibility of starting known therapies that offer some survival advantage, such as Provenge and Xofigo which I believe could be given sequentially. If financially possible consider Lu 177 PSMA/Ac 225 treatment abroad. There are clinical trials for PSMA PET/CT, I gave you the links last week.

I tend to agree with you on this one. I'm feeling we need to get these bone mets more under control before trying unproven therapy. I have saved the clinical trials you sent. The issue for us is they require tissue sample which we have not been able to get yet that is also something our doctor is looking into. Thanks for your response!

I hope you will be able to overcome the problem of the pathology requirement and get the PSMA scan.

I am BRCA2+. Have been on Olaparib for 10 mos, and so far for the last 8 my PSA has been undetectable. My Oncologist was able to get off-label approval for me to get the drug.

Does the trial randomly select who gets the meds, or does everyone get them?

Everyone gets both drugs. What are your side effects on olaparib? Not that side effects would matter if I brings your psa to undetectable!

Some anemia with low RBC count, causing additional fatigue above ADT side effects. Also got partial blood clot in leg, resulting in being put on blood thinner Eliquis. All worth it!

My husband is in an Olaparib trial at mdandersen. He has had bad weakness, does not have Barca mutation. They had to reduce dosage due to fatigue. Hemoglobin low, psa 0. We are going back to Houston next week for scans.

Where are u being treated.

Was there another drug in combo with olaparib? Psa 0 is worth fatigue!

I took Lynparza for about 9 months after Guardant360 liquid biopsy revealed ATM defect. No SEs that I can recall.
