Lower doses of Lupron?

Hello! A reminder: I have recurrent prostate cancer after

radiation and two years of Lupron. The Lupron drove me

nuts. Fatigue & depression made me a zombie. Now, after

tests & scans showed it's local, not metastatic, my docs ruled

AGAINST curative therapy (cryo, brachy, hifu, etc.) because cancer

was outside gland. Felt they couldn't get it all, and poking around

might spread it. Only prescription: Lupron (Eligard) again!

I've read that in Europe, they often use much lower

doses of it than we do -- sometimes only half

(11.25 mg for 3-month shot, etc.) Mainly because they

use a testosterone-based standard of effectiveness as

well as PSA. A study by Dr. Nicholas Sanfilippo at David H.

Koch Center in NY said studies show that Lupron "may be

efficacious for longer" than we thought; testosterone-based

method can lead to "lower dose levels" and hence fewer

side effects. Found other references to "dose adjustments"

in Europe as well.

Showed these to my docs. Reply:

"We don't do that here. Give us the EVIDENCE

that lower doses actually WORK - and benefit patients

as well as our doses do."

So: Do any of you have any knowledge about European

practices -- especially the lower doses?

Also, Europeans often start treatment with Casodex-type

drug; use till it fails (maybe a year or two), THEN start

Lupron. Any of you heard of this?

Just had my first 3-month shot of Eligard (regular

U.S. dose, 22.5 mg). I'm writing to cancer centers

in Europe (ESMO, EMA), plus global programs of

U.S. outfits. But you guys are a lot smarter than

I am about having -- and getting -- info. Please

let me know if you can help keep me from becoming

a Lupron zombie again!

Your doctor is right. Maximal testosterone suppression works better than less. You can try 150 mg/day of Casodex + 10 mg/day of tamoxifen instead. Not quite as good, but you might like the side effects better.

Just curious, was your prostate surgically removed before your radiation and Lupron treatments? I also, by the way, went through the affects you described from Lupron and was helped a lot by taking 10 mg per day of Lexapro.

I have Lupron injection monthly. 1/2 dose of normally prescribed. 1st injection, T dropped immediately from 900(Xtandi driven) to 17. 4 injections later T has remained about 9.

The downside,,,,PSA continues to climb indicating immediate hormone resistance. However with T at 9,,,likely has nothing to do with 1/2 dosage.

I was scrolling through my old “saved” posts and came across yours!!

I (unfortunately) can speak MUCH more inelegantly since the time you posted!!

I AGREE WHOLEHEARTEDLY IN US OVERDOSING!!!!!

In a nutshell;

I had my last 3mo injection on Nov 9th 2018 after an 18mo treatment regiment. On May 9th 2019 I was still full blown ADT with my “T” 19 (in lieu of the constant 18) and PSA 0.0?. So, this is 4 months past the point where I would be requiring another injection so I be continuing treatment.

“Lets give it another 6 months” he said which brings us to now!!!

Add this to the fact that half way through my treatment, when experiencing debilitating side effects, I missed 6 weeks due to the firing of my Dr and locating his replacement. Bloodwork upon securing a replacement Dr showed that ADT was not compromised. In his words “ a month or so doesn’t matter!!

So, once again, this brings us to now!! Blood work due next week with Dr app on 11/14. Based upon the unrelenting side effects, I am not expecting much of a rebound....still.

I’ll likely post if Murphy imposes his / her law. Meanwhile, do you have an update since April!! Are you on the “suggested” dosage regiment?

Ps: In case someone is paying attention and in the interest of clarity, I had stated June 9th in lieu of May 9th in many other posts. That was a mistake on my part!!

12/19. FYI..... greater than one year after my last 3mo 22mg Eligard injection, and I am just now clawing my way out of ADT with a T @100 and still no PSA. Can you say overdosed? I thought you could!!

Further proof that US standards for leuprolide drugs are the same as mostly all other prescription drugs verses other countries. Big Pharm owns our government officials and lobby to stay that way to continue to cash in at our expense (more than one meaning)!
