While reviewing the 3tmp MRI with the doctor he said that the Tumor was encapsulated albeit pressing a bit onto the bladder . Now the report is out and it reads : Region of interest located at the left anterior transition zone measuring 1.5 x 1.6 cm at the 12- 2 o’clock position Pirads 5 with 3/5 Extracapsular extension. When I questioned him he said that there is really no capsule around the prostate and that’s just the way we have to explain what we see and write the report ....I’m reading that it means the cancer is out of the prostate ....thoughts anyone?

It means the cancer is out of the prostate. Since the extension has grown to the bladder it could be classified as T4.

Sorry, my tumor was pressing on the edge and it was out of the pocket.

They classified it as T2W ...whatever that means ..

A range from 1-4 . I was T4 most aggressive so It could have been ever worse for you. They told me the horse is out of the barn . You can do much about your condition . Staying calm and dealing with it as it comes will serve you well .. still it ain’t no joke .. but doable .. you4 life isn’t over , just ... altered

"Pirads 5 with 3/5 Extracapsular extension." So there is extracapsular extension and that is not a T2. Only this extension can be "pressing a bit onto the bladder".

That is true: there is no capsule surrounding the prostate.

All of the urologists and oncologists I've talked to use the word "capsule" to describe the outer boundary of the prostate.

Microanatomy

The prostate contains two main types of tissue: exocrine glandular tissue and fibromuscular tissue.

Exocrine glandular tissue in the prostate is epithelial tissue specialized for the secretion of the components of semen. Most of the prostate is made of exocrine glandular tissue, as the prostate’s primary function is the production of semen.

Fibromuscular tissue is a mixture of smooth muscle tissue and dense irregular connective tissue containing many collagen fibers. The collagen fibers of the tissue provide strength to the tissue while the smooth muscle permits the tissue to contract to expel fluids. Fibromuscular tissue forms the outermost layer of the prostate and the tissue surrounding the urethra.

So whatever one wants to call it, the exterior of the gland is rich in collagen fibers that are stronger and tougher than the interior.

All of my doctors describe this condition, with tumor extending beyond the outer boundary of the prostate, as "locally advanced disease."
