Hello there Mitch is my name. I wondered how to determine the Gleason score as I have readings of 7-10. Do we average it?

The gleason score should read with two numbers such as 3+4 which is 7 Or a (4,3) which is also a seven but with the higher grade 4 first it would be considered the breaking point for high risk. The highest most aggressive Gleason is 10.It should be on the biopsy report. a 7 to 10 is a broad range , and should be clarified. You can request that the specimen (which whoever did the biopsy has in wax) be sent for expert review to John Epstien at Jons Hopkins hospital for an expert review, The Gleason score is very important.

It was weird as my Dr called me on a Sunday to tell me I had a very aggressive cancer that was spreading thru my body. Fortunately it has not shown up anywhere else yet.

I say that’s a fine testament towards your doctor to call you on a Sunday. We hope for

I assume that was the results of the biopsy - so there were areas that could be GL7 (3+4) is not much concern to a GL10(5+5) which would mean really malformed cancer. What was your PSA history, any symptoms, how is your overall health?

Yea, those darn MDs :-) - prolly saving your life tho. I had no symptoms and a GL7(4+3), PSA @ 840, started ADT(Lupron and Casodex), had 15 taxotere chemos in 2015. Changed diet and added supplements and PSA at 0.1 and Testosterone at 24. Stopped ADT after 30 months.

Best to you on upcoming treatments

If you have any Gleason 10, it is a very serious matter.

You probably would want to find a really good medical oncologist, preferably a genitourinary medical oncologist (one who specializes in prostate cancer). Don't know where you live, but seek out top doctors, preferably at a Center of Excellence. The level of disease you have is beyond what a urologist should be treating. That is what I would do, and what I in fact did, and so far it has saved my life.

Important...make sure they save those biopsies. I've been turned down for two trials because my urologist lost my tissue.

I'm partial to the Sidney Kimmel Clinic at Johns Hopkins. I too had a high Gleason. I'm a retired food scientist and believe as they do that prostate cancer has a 90% dietary link.
