OHI form? Can anyone help?

Does anyone know anything about an OHI form needed for my 1st grade child? I was told that we need to complete an OHI form? If anyone has light to shed on this subject please I would appreciate all the help . Thank You

Emcee74- I know this process is all overwhelming.. Do you think it could be the survey to evaluate your son for a Individual Educational Plan (IEP)/504 That is the only reason they would be talking about that.

You could ask the person who gave it to you. What is the survey?

Last I knew OHI stands for other health impairment. When my child was assessed for a learning disability at about 4 or 5 years old ( my son is 19 now) he was eligible for special education under OHI because he had a medical condition and just barely had a learning disability. As the years went by his learning disability became more apparent and in 5th grade his eligibility was SLD; Specific Learning Disability, which was an auditory processing but at 18 they said they think he has ADHD- inattentive. So the moral of the story here is get second and third opinions and if you don’t understand something ie an assessment such as OHI then you have the right to get a outside specialist to evaluate but you have to be persistent! I learned all of this when my son turned 18 wish I knew better!
