So I’ve been on Rux for a while which I’m pleased to say is controlling my blood levels.
I have been taking Teva brand Amitriptyline as pain control for many years now. Unfortunately this has not been available for a while and the other brands don’t control the pain for me.
I have just been prescribed Duloxetine 30mg to take before bed. I will be asking the specialist once I have been referred back to London but for now I start this new medication on Monday.
Anyone else taken Duloxetine with or without Ruxolitinib?

I’ve never heard of taking Anagrelide for pain relief. I used to take it to control my platelet counts, but it had too many side effects and I couldn’t handle it. What kind of pain are you having? My body seems to be increasingly in pain, too. And I’m wondering about this Duloxetine for me. I’ve never heard of it. I have fibromyalgia and have always had muscle pain, and now with MF, my pain seems to be increasing, especially in my upper arms and my neck. Just can’t figure out the difference between MF pain and pain from fibromyalgia.
The Amitriptyline was aimed at Neurological pain which I have experienced throughout my body. More recently I believe it is musculoskeletal pain. Everything seems dry and crunchy 🤓 I think the new medication is aimed at neurological pain. Once referred I will ask the specialist if there is anything in addition to Rux to help control bone and joint pain.
Duloxetine does have an indication for neuropathic and musculoskeletal pain (and fibromyalgia). It is an SNRI also used as an anxiolytic and antidepressant. It seems like a pretty good choice to try given what you have described. I have never used it myself, but would consider in the circumstances you have described.
I checked ePocrates and did not see any interaction between Duloxetine and Ruxolitinib. There is however an interaction with aspirin (expected with a SNRI).

monitor bleeding s/sx: combo may incr. risk of GI or other bleeding, incl. life-threatening (additive effects, antiplatelet effects augmented by inhibition of platelet serotonin uptake)
This warning does not mean to not consider the Duloxetine if taking aspirin, but does indicate a need to monitor your bleeding response when taking them together.

Well I took one 30mg last night, and boy what a night. Feeling Very sick, stomach cramps, restless legs, headache, coughing. Not sure I want to push through this time. Considering cold turkey and paracetamol till I hear from Guy’s 🥸 Might revisit the other brands of Amitriptyline to see if they help. I had tried two out of the three brands years ago but no record of which ones.

WOW! You hit the trifecta of side effects. Sorry to hear you had such immediate adverse reactions. On to something else. Hope you find something that works.
Thank you, Hunter, again for your response. Many doctors over the years have asked me what I take for my fibromyalgia. I really don’t take anything, but I feel that now that I’m getting older, and I have the double whammy of also having myelofibrosis, I might look into this drug. Thanks again. I hope you are doing well.
Hope you find something that helps with the fibromyalgia. Recently I have been experiencing some intermittent pain in my toes that might be erythromelalgia. Really don't know what it is for sure. going to discuss it with the MPN Specialist when I see him in May. Might be looking to change my treatment plan at that point. We will see what happens.
My left big toe hurts a lot lately when I follow through with my foot to walk and also my left thumb hurts. I am going to ask specialist about it in July.
