Xtandi after Zytiga

Quick question... anyone have success on Xtandi after failure of Zytiga? (8 months)

Yes, my brother did. I do not recall the PSA number, but it reduced by 50% by the 2nd month. (The first month there was no significant change). After the 2nd month, it continued to drop. My brother died from unrelated causes and therefore, I do not know how effective it would have been going forward.

Just do not give up on it during the 1st 2 months. The doctor was surprised that it dropped, but the literature explained that, in fact, it can take that long. Thank you for your condolences.

I did not get a durable remission maybe 4 months. Plan to rechallenge after chemo or immunotherapy.

As always I value and appreciate your advice. We have followed through on your advice in other matters and getting some results from otherwise resistant doctors. We will pose this question to our doctor as my brother is very interested in BAT along with Xtandi. She has put him on Xtandi even though Zytiga has failed and that is why I asked the question. I am understanding the significance of the gene splice which you brought to my attention earlier. My brother has previously done chemo and although he went through easily he absolutely does not want to do it again. We have contacted foundation one and have that in the works. Thank you again for your answer we definitely are going to bring this up with his doctor

It usually works for a short time, but it is much better to do chemo in between:

Thank you so much Tall Allen for your response and the link. I believe my brother did very well on Docetaxel. He did it early on and had no more spread visible on scans until recently. Certainly something to consider.

Has anyone tried this the other way around i.e. xtandi then Zytiga? I have used xtandi and had a poor response. Then put on Jetvana and now doing Lu-177. I was wondering what i do next.

Yes I did quite well on xtandi after zytiga. As I recall I had Provenge in between.

Thank you so much Magnus! My brother just completed Provenge. This makes me hopeful. Very interesting what Nalakrats has to say as well. So many new approaches involving immunotherapy. Thank you so much for your reply.

I will bring up Dr Denmeade’s name to my brothers Dr and see if she will contact him to discuss this. If she says this is not SOC then I will contact him myself. This is promising and time is of the essence. I am finding that you have to be your own advocate. Thank you so much for educating all of us here and taking complicated material and making it understandable.

There is a trial that combines BAT with nivolumab, a checkpoint inhibitor. It’s at Hopkins, Dana Farber and UCSF. No placebo arm, everyone gets the treatment. I read an interview with the principal investigator, Dr Antonarakis, and he’s very pleased with the initial results- better than either treatment alone. clinicaltrials.gov/ct2/show...

Thanks for your reply BruceSF. We spoke to his doctor at Dana-Farber in regards to that clinical trial however she says he does not qualify. When I look at the eligibility qualifications it says you have to have a PSA over 2.0 and must have a soft tissue metastatic lesion so they can get a biopsy. My brother has several Mets in his spine hip and pelvis but nothing in soft tissue. Damned if you do damned if you don’t.

I also got 8 months Pca suppression from got Zytiga. But in Australia, our Medicare system won't allow Pca sufferers to move to Xtandi after Zytiga fails. But if you have chemo after Zytiga fails, the Pca may be re-sensitized to allow either Zytiga or Xtandi to be used again.

I had Lu177 after chemo failure and began Xtandi during Lu177, and Psa went low than it otherwise may have but Psa is rising fast now so the Xtandi maybe gave about 12 months Psa suppression. I am continuing to take it because docs think its boosting PsMa expression so when I get more Lu177 in 3 weeks, Xtandi should boost its effect. I am also to be given

Veyonda to boost PsMa expression before getting more Lu177.

My ADT continues with monthly injections.

One thing seems certain, taking Zytiga and Xtandi together seems a very bad idea according to data about that I read.

That combo of Veyonda with Lu-177 appears to do much more via the immune system than simply enhancing PSMA expression. Good luck with that and please let us know.

The Veyonda I'm getting is free on compassionate grounds, because its not approved fully here yet in Australia. I am not sure what all of its function is, but I trust my doc on that, and if there are any bad side effects, I'll just stop it, but my doc is confident.

It was a fine clear winter day here, about 12C max. I know Sunday is supposed to be a day of rest, and a day to contemplate God, but you'd think that if he needed more of my attention, he'd send a sign, but it has not sent one for at least 60 years, so I don't feel any need to bother him because I ain't needy or wanty, and I cope OK without God, so I cycled out to end up doing 85km journey at good speed, and I felt well.

If my immune system were more able to recognize my Pca was my body's biggest enemy, and attacked and killed my cancer like it kills a myriad of other unwanted bad cells, then my Pca would soon be gone.

Sumwun@somewear said I ought to get a marrow transplant using some of Germaine Greer's highly feminist cells, which Femcan Corp has cultured up in test tubes.

They said I could try them, but there's every possibility that the major side effect is to transform me into a very grumpy old lady, and I'd be arguing too much with my fine male doctors.

Nal is this a clinical trial? I know you said this combination was extremely new as a paper was just published on June 20. It looks like this trial started in 2018 so I’m assuming it’s different? BruceSF mentioned something similar in his reply. Dr. Antonarskis is involved in the clinical trial at John Hopkins. Didn’t know if this is the same or something different. I am hoping it is something different as my brother did not qualify for that clinical trial at Dana-Farber as he does not have soft tissue metastasis available for a biopsy. He has bone Mets in his spine hip and pelvis.

Yes. Failed Zytiga and now been on Xtandi/Lupron for 41 months. PSA less than .1 and all spots are remiss. They call it progression free survival. Told me Xtandi would only work for up to 24 months for me having had tons of chemo already. I’m blessed.

Yes, I did for a short while. PSA went down slightly the second month then started marching upward. I started taxatere 11 days ago.

Thank you John. He’s a great guy and way too young to be going through this He has a wonderful wife and family. Sometimes going through this is so overwhelming when you’re just not feeling good and scared as well. It’s easier for me to step in and support him where I can. I go with him back and forth to Dana-Farber at least until Covid hit. I try to be prepared to speak to his doctors and take notes as well. I spend a lot of time talking with him and discussing further treatments, clinical trials and getting through the myriads of information that is so hard to decipher at times. I couldn’t imagine not being there for him.

Hi, I was first diagnosed with PC in 1999. I have had many different treatments. In 2013 my Oncologist diagnosed Stage Four. And started me on Zytiga 1000 mg/day. I have also be on Lupron for at least 10 years. Zytiga gave me no problems. PSA floated around from 4 to 11. In Jan. 2020 with PSA at 19, the Dr. dropped Zytiga and started Xtandi 1000 mg per day. My opinion is that Xtandi is a terrible drug. I developed every side effect listed as possible. I was really sick for two months. No change in PSA. My Dr. switched me back to zytiga. I feel much better, however my PSA is stuck between 19 and 20. He as just added a 7 month course of Radium 223 (Zofiga) Liquid radiation injection once monthly. That lays me out for a couple of days. By comparison I have a friend that is on the same path. He has been on Xtandi for 2 months and is suffering terribly. He has all of the stated "possible side affect." However his PSA dropped from 19 to1.4. His Doctor refused to believe that the Xtandi is the problem. So he is going in for a full upper and lower GI investication including Endoscopy. Advise to all: research thoroughly every drug and treatment your Dr. wants to use. We all need a Doctor to watch the Doctor. God Bless.

Sheba215, Thank you for your reply. Had you been on Zytiga Since 2013? If so that was a good run! sorry Xtandi gave you so many side effects. I’m really hoping my brother doesn’t have those side effects that you had. He was fine on Zytiga. I guess time will tell. Best of luck with the radium 223.

Hello My name is Helen ,my late Husband was on Zytiga ,he took it for over a year ,from beginning it worked ,my advise is until it is o point take it soon it goas up it does not work it will rise and rise and keel all your muscles and take awey all your energie ,we kept asking the oncologist to stop it take him of ,but no he did not it went up to twelve he could not stand up sit up and barly turned I took him of and with help idea from my niece I asked his doctor by then he becam poliatic patient , his specialist told me he has 5-10 days leftto put him on I v because he was dehydreted ,of corse I had him always at home and we put him on IVE and hamp oil CBD pure Kannaway Pure gold ,day by day his energy slowly came back ,but Cancer is hard to beat , he lived 13 more month ,I don't recommend Zytiga, radiation is aption ,if anny one is diognosd with prostet Cancer have a surgery and if needed after have radiation ,no kimo ,many that I know had that and there steel with us with little or no problem ,I wish you all the best my husband was 82 years of age I lost him in march ,miss him so much :(
