I tend to ruminate over thoughts of possibly having offended people or of having hurt their feelings. I tend to apologize profusely and have always been told, up to now, that I have not been offensive or hurtful. The problem is that I am 150% certain that my tone was inappropriate when I recently responded to a question someone asked me. This person has done nothing but help me, so I am absolutely sick over it - literally, I've thrown up twice. My question is, should I apologize?

No, you should not apologize. That's feeding an obsession and making it stronger. BUT...you can send this person a friendly card and say how much you appreciate them IF you can do it without apologizing 😊. Everyone loves to get mail. Be friendly and unapologetic. A good friend will either speak up or let it go if you say something in the "wrong" tone of voice.
